静脉用药调配中心多环节质量控制应用实践

Application practice of multi-link quality control in pharmacy intravenous admixture service center

目的为有效提高静配中心的工作效率,降低差错率及药品损耗率,进一步保证临床患者用药安全。方法回顾性分析2017年1月1日～12月31日我院静配中心工作量、差错和药品耗损的数据记录,对各工作环节进行梳理探究,制定相应措施,如贴签、摆药差错防范措施、调配差错防范措施、成品输液发放差错防范措施、停医嘱退药差错防范措施、审方差错防范措施等,观察实施多环节质量控制措施前后静配中心工作效率、差错率、药品损耗率的改善情况。结果静配中心实施该方案后调配总量较对照组增长20.78%,差错数较对照组减少53.66%,药品损耗量较对照组减少87.70%,两组数据差异有统计学意义(P <0.05)。结论加强各环节质量控制,定期进行专业技能培训及考核,增加工作人员知识储备,有利于提高静配中心工作效率,降低差错率及药品损耗率,值得医院推广实施。

Objective To effectively the work efficiency of pharmacy intravenous admixture service center and reduce error rate and drug loss rate to further ensure the safety of clinical medication of patients.Methods Data records of the workload,errors and drug loss of the pharmacy intravenous admixture service center in our hospital from January 1,2017 to December 31,2017 were retrospectively analyzed.The various work links were investigated and explored and corresponding measures were formulated such as labeling,preventive measures for error placement,prevention measures for admixture error,prevention measures for finished infusion delivery error,prevention measures for drug withdrawal error on doctor’s order and prevention measures for checking prescriptions errors.The improvement of the working efficiency,error rate and drug loss rate of the pharmacy intravenous admixture service center before and after the implementation of multi-link quality control measures was observed.Results After implementation of multi-link quality control measures in pharmacy intravenous admixture service center,the total amount of admixture increased by 20.78% compared with the control group,the number of errors decreased by 53.66% compared with the control group,and the drug loss decreased by 87.70% compared with the control group.There were significant differences between the two groups(P<0.05).Conclusion Strengthening the quality control of all links,regularly carrying out professional skills training and assessment and increasing the knowledge reserve of staff,which is conducive to improve the work efficiency of pharmacy intravenous admixture service center and reduce the error rate and drug loss rate.It is worthy of promotion and application in the hospitals.
