柳州市围产期孕妇B族链球菌感染率调查及耐药分析

Investigation and drug resistance analysis on infection rate of streptococcus group B in perinatal pregnant women in Liuzhou

目的调查柳州市围产期孕妇B族链球菌感染率及耐药分析。方法通过随机整群抽样的方法收集我市三家医院2016年6月～2018年3月15796例孕35～38周孕妇,采用细菌培养的方法分离并用法国梅里埃VITEK-2 compact全自动微生物鉴定仪进行签定。用纸片扩散法(K-B法)进行药敏试验,参照2017版美国临床和实验室标准化协会(CLSI)标准判定药敏结果,采用WHONET5.6软件进行统计分析。结果 15796例围产期孕妇中有1132例分离出B族链球菌,感染率7.17%,药敏试验显示,B群链球菌对氨苄西林、头孢呋辛、头孢唑啉、头孢曲松都保持很高的敏感度。结论围产期孕妇阴道和直肠均为B群链球菌的寄殖部位,两部位的标本应同时送检以提高其检出率;青霉素仍可作为孕妇治疗B群链球菌感染的首选治疗药物,B群链球菌阳性的孕妇应立即进行抗菌药物干预治疗,以保证胎儿健康。

Objective To carry out investigation and drug resistance analysis on infection rate of streptococcus group B in perinatal pregnant women in Liuzhou.Methods 15,796 pregnant women with pregnancy for 35-38 weeks in three hospitals in our city from June 2016 to March 2018 were collected according to the random cluster sampling.It was isolated by bacterial culture and identified by the French Mérieux VITEK-2 compact automatic microbial identification instrument.The drug sensitivity test was carried out by the disk diffusion method(K-B method).The drug sensitivity results were determined according to the 2017 edition of the American Clinical and Laboratory Standards Institute(CLSI)standards and statistical analysis was performed using WHONET 5.6 software.Results Among 15,796 perinatal pregnant women,1132 were detected with streptococcus group B and the infection rate was 7.17%.Drug sensitive test showed that streptococcus group B was highly sensitive to ampicillin,cefuroxime,cefazolin and ceftriaxone.Conclusion Both vagina and rectum of perinatal pregnant women are the colonization sites of streptococcus group B and samples from both sites should be sent for examination at the same time to improve the detection rate.Penicillin can still be used as the first choice of treatment for pregnant women infected by streptococcus group B.Pregnant women with positive streptococcus group B should immediately receive the treatment with antimicrobial intervention to ensure the health of fetus.
