新辅助化疗后保乳手术治疗乳腺癌的临床效果

Clinical effect of breast conserving surgery after neoadjuvant chemotherapy in the treatment of breast cancer

目的探讨新辅助化疗后保乳手术治疗乳腺癌的临床效果。方法收集2014年7月～2016年8月来我院接受保乳手术治疗的48例早期乳腺癌患者的临床资料。按照术前是否接受新辅助化疗分为观察组24例和对照组24例。对照组:保乳手术治疗。观察组:新辅助化疗后保乳手术治疗。观察患者手术情况、疗效、生活质量、术后并发症发生情况。结果观察组手术时间[(43.91±7.25)min vs(79.95±8.66)min]、术中出血量[(131.47±16.24)m L vs(245.81±18.77)mL]、术后住院时间[(9.23±1.28)d vs(16.17±3.49)d]均低于对照组(P <0.05),两组患者疗效比较,差异无统计学意义(P> 0.05),观察组手术前和手术后1年生活质量对比,差异无统计学意义(P> 0.05);对照组手术后1年生活质量低于手术前(P <0.05)。观察组术后并发症总发生率(16.67%vs 58.33%)低于对照组(P <0.05)。结论新辅助化疗后保乳手术治疗乳腺癌效果较好,患者手术时刻短、术中出血量少,术后住院时间短,生活质量未受到明显影响,术后并发症较少,临床应用价值较高。

Objective To explore the clinical effect of breast conserving surgery after neoadjuvant chemotherapy in the treatment of breast cancer.Methods Forty-eight early breast cancer patients who received the breast-conserving surgery in our hospital during July 2014 to August 2016 were collected as an observation group and a control group,with 24 patients in each.The two groups both received the breast-conserving surgery,and the observation group was given neoadjuvant chemotherapy treatment before the surgery.Results The operation time[(43.91±7.25)vs(79.95±8.66)],intraoperative blood loss[(131.47±16.24)vs(245.81±18.77)]and postoperative hospitalization time[(9.23±1.28)vs(16.17±3.49)]in the observation group were lower than that in the control group(P<0.05).The curative effect of the two groups was similar and there was no statistical difference(P>0.05).There was no statistical difference in the quality of life of the observation group before and one year after operation.Learning significance(P>0.05);the quality of life in the control group one year after operation was lower than that before operation(P<0.05).Total incidence of postoperative complications in the observation group(16.67%)was lower than that in the control group(58.33%)(P<0.05).Conclusion Breast conserving surgery after neoadjuvant chemotherapy is effective in the treatment of breast cancer.The patients have short operative time,less intraoperative blood loss,short postoperative hospitalization time,unsignificantly affected quality of life and less postoperative complications,which is high clinical valuable.
