体外冲击波联合复方金钱草颗粒、哈乐在治疗经皮肾镜碎石术后残石的临床疗效分析

Analysis on clinical curative effect of extracorporeal shock wave combined with compound Jinqiancao Granules and Harnal in the treatment of residual stones after percutaneous nephroscope lithotripsy

目的探讨体外冲击波联合复方金钱草颗粒、哈乐在治疗经皮肾镜碎石术后残石的临床疗效及应用价值。方法选取我院2012年8月～2018年1月行经皮肾镜碎石取石术后残留结石患者150例。按随机数字表法将其分为A组(体外冲击波联合复方金钱草颗粒、哈乐),B组(体外冲击波联合复方金钱草颗粒),C组(体外冲击波,即对照组),每组各50例。比较三组在体外冲击波碎石后第1周和第4周时残石排出情况、残石清除率、术后并发症发生率及OABSS评分等方面的差异,评估临床疗效。结果三组患者在体外冲击波碎石后第1周和第4周时的残石清除率、术后并发症及留置输尿管双J管OABSS评分情况比较差异均有统计学意义(P <0.05)。A组在第1周和第4周时的残石清除成功率均明显高于B组及C组(P <0.05)。A组在第1周和第4周时的术后并发症及留置输尿管双J管OABSS评分均显著低于B组及C组(P<0.05)。结论体外冲击波联合复方金钱草颗粒、哈乐在治疗经皮肾镜碎石术后残石是安全有效的,具有残石清除成功率高,缩短残石排出时间,术后并发症少及临床效果显著,留置输尿管双J管OABSS评分低,值得临床上的推广和应用。

Objective To explore the clinical curative effect and application value of extracorporeal shock wave combined with Compound Jinqiancao Granules and Harnal in the treatment of residual stones after percutaneous nephroscope lithotripsy.Methods 150 patients with residual stones who underwent percutaneous nephroscope lithotripsy in our hospital from August 2012 to January 2018 were selected.They were divided into group A(extracorporeal shock wave combined with Compound Jinqiancao Granules and Harnal),group B(extracorporeal shock wave combined with Compound Jinqiancao Granules)and group C(extracorporeal shock wave,the control group)according to the random number table method,with 50 cases in every group.The differences in excretion of residual stones,clearance rate of residual stones,occurrence rate of postoperative complications and OABSS score between one week and four weeks after extracorporeal shock wave lithotripsy were compared among three groups.The clinical curative effects were evaluated.Results The clearance rate of residual stones,postoperative complications and indwelling ureter double-J tube OABSS scores between one week and four weeks after extracorporeal shock wave lithotripsy of patients in three groups all had statistical difference(P<0.05).The successful clearance rates of residual stones in group A on one weeks and 4 weeks after extracorporeal shock wave lithotripsy were significantly higher than those of the group B and group C(P<0.05).The postoperative complications and the indwelling ureter double-J tube OABSS scores in group A on one weeks and four weeks were significantly lower than those in group B and group C(P<0.05).Conclusion The extracorporeal shock wave combined with Compound Jinqiancao Granules and Harnal in the treatment of residual stones after percutaneous nephroscope lithotripsy is safe and effective.It has high success clearance rate of residual stones,shorter excretion time of residual stones,fewer postoperative complications,significant clinical effect and low indwelling ureter double-J tube OABSS scores,which is worthy of clinical promotion and allocation.
