L-OHP联合5-FU-LV与HCPT联合5-FU-LV化疗方案对可切除胃癌术后预后的研究

Prognosiso bservation of chemotherapy regimens in postoperative resectable gastric cancer with oxaliplatin and hydroxycamptothecine plus 5-FU-LV

目的:探讨奥沙利铂(L-OHP)联合5-FU/LV与羟基喜树碱(HCPT)联合5-FU/LV化疗方案对可切除胃癌术后预后的影响,以期寻找胃癌切除后的最佳化疗方法,降低胃癌复发率,提高患者生存时间和生活质量。方法:将120例腹腔镜胃癌根治术后患者分为A、B两组,每组60例。其中A组受试者采取L-OHP联合5-Fu/LV化疗方法;B组采取HCPT联合5-Fu/LV化疗方法,治疗4个月后对两组患者化疗后治疗效果、生存率及不良反应进行统计分析。结果:A组受试者治疗后的总有效率约为46.67%,明显高于B组的26.67%,两组间差异有统计学意义(P<0.05);两组受试者血液毒性不良反应发生率以及手术并发症发生率差异无统计学意义(P>0.05);A组受试者消化道不良反应发生率高于B组(P<0.05);A组受试者无疾病生存时间、18个月无疾病生存率以及18个月总生存率均明显高于B组,两组间差异存在统计学意义(P<0.05)。结论:对于腹腔镜胃癌根治术的患者,L-OHP联合5-Fu/LV化疗方案优于HCPT联合5-Fu/LV化疗方案。

Objective:To investigate the prognosis of chemotherapy regimens in postoperative resectable gastric cancer with oxaliplatin(L-OHP)and hydroxycamptothecine(HCPT)plus 5-FU-LV,looking for the best chemotherapy after gastric cancer removal,reducing the recurrence rate of gastric cancer and improving the patient's survival time and quality of life.Methods:120 patients with laparoscopic radical gastrectomy were randomly divided into A,B two groups,each group of 60 cases,group A of subjects were given oxaliplatin plus 5-FU-LV,group B of subjects were given hydroxycamptothecine plus 5-FU-LV.After four months of treatment,analysis the treatment effect,survival rates and adverse reactions of the two groups.Results:The total efficiency of group A therapy was approximately 46.67%,significantly higher than the 26.67%in group B,and the difference between the two groups was statistically significant(<0.05);There was no significant difference in the incidence of blood toxicity and the incidence of surgical complications in both groups(<0.05);The incidence of adverse reactions was higher in group A than in group B(<0.05);The disease free survival,18 months disease free survival rate,total survival rate in group A were significantly higher than group B,the difference between the two groups was statistically significant(<0.05).Conclusion:For patients with laparoscopic gastric cancer radical surgery,L-OHP plus 5-FU-LV chemotherapy is superior to HCPT plus 5-FU-LV chemotherapy.
