脑卒中患者早期血清BDNF、Hs-CRP及IGF-1含量与脑卒中后抑郁的关系探讨 被引量:18

摘要 目的探讨脑卒中患者早期血清脑源性神经营养因子(BDNF)、超敏-C反应蛋白(Hs-CRP及人胰岛素样生长因子-1(IGF-1)含量与脑卒中后抑郁的关系。方法收集唐山市开滦总医院2016年3月至2018年2月收治的100例脑卒中患者为研究对象,根据患者是否发生抑郁分为脑卒中后抑郁组(观察组)和脑卒中后非抑郁组(对照组),观察组43例,对照组57例。检测患者入院后第2天BDNF、Hs-CRP及IGF-1水平,比较两组患者美国国立卫生研究卒中量表(NIHSS)评分、活动能力量表(ADL)评分及改良Rankin量表(mRS)评分并评估BDNF、Hs-CRP及IGF-1水平和抑郁的相关性。结果和对照组相比,观察组患者BDNF水平更低(P<0.05),Hs-CRP及IGF-1的水平更高(P<0.05),NIHSS及mRS评分更高(P<0.05);ADL评分更低,差异有统计学意义(P<0.05)。Hs-CRP水平与抑郁评分呈现正相关(r=0.736,P<0.05),IGF-1水平与抑郁评分呈现正相关(r=0.752,P<0.05),BDNF水平与抑郁评分呈负现相关(r=-0.692,P<0.05)。结论脑卒中患者早期血清BDNF、Hs-CRP及IGF-1水平的异常可能提示患者发生抑郁的风险更高。 Objective The objective of this article is to investigate the relationship between the serum level of brain-derived neurotrophic factor( BDNF), high sensitivity C-reactive protein( hs-CRP )and insulinlike growth factors-1(IGF-1) with post-stroke depression in stroke patients. Method A total of 100 patients with stroke in Kailuan General hospital from March2016 to February 2018 were divided into two groups according to their HAMD's scores. One was PSD group(observation group, n=43), the other was non-PSD group(control group, n=57). The serum level of BDNF, Hs-CRP and IGF-1and the scores of NIHSS, ADL and mRS were compared between the two groups. To evaluate the correlation between serum level of BDNF, Hs-CRP and IGF-1with depression. Results The level of BDNF was lower in observation group and the level of Hs-CRP and IGF-1 was higher when compared with control group( P<0.05). The scores of NIHSS and mRS were higher and scores of ADLwere lower in observation group( P<0.05). In addition ,the serum BDNF and IGF-1 level had a positive correlation with depression (r=0.752, P<0.05), the serum Hs-CRP level had a negetive correlation with depression(r=-0.692, P<0.05). Conclusion The abnormal changes of BDNF, Hs-CRP and IGF-1 serum level in stroke patients may indicate a higher occurrence rate of PSD.
