胆通颗粒在胆囊切除联合胆总管切开T管引流术后的应用效果

Application efficacy of Dantong Granules after cholecystectomy combined with choledochotomy and T-tube drainage

目的观察胆通颗粒在胆囊切除联合胆总管切开T管引流术后的应用效果。方法 72例胆总管结石并发急性梗阻性化脓性胆管炎(AOSC)患者择期行胆囊切除联合胆总管切开T管引流术治疗。将其随机分为观察组与对照组,每组36例。两组术后均给予常规治疗,观察组加用胆通颗粒治疗。比较两组患者治疗有效率、术前1 d及术后第7天的炎症指标、术前1 d、术后第7天和术后第12天的肝功能指标及术后第7～12天的胆汁流量。结果两组治疗总有效率比较,差异无统计学意义(P> 0. 05)。术后第7天,两组白细胞计数、C-反应蛋白水平均低于术前,且观察组均低于对照组(均P <0. 05)。观察组术后第7天总胆红素及ALT水平、术后第7天及第12天直接胆红素水平均低于对照组(均P <0. 05)。观察组术后第7～12天胆汁引流量多于对照组(P <0. 05)。结论胆通颗粒治疗胆囊切除联合胆总管切开T管引流术后患者,可增加胆汁引流量,减轻炎症反应,改善肝功能,疗效确切。

Objective To observe the application effect of Dantong Granules afteRcholecystectomy and combined with choledochotomy and T-tube drainage.Methods Seventy-two patients with choledocholithiasis complicated with acute obstructive suppurative cholangitis were treated with selective cholecystectomy and combined with choledochotomy and T-tube drainage.The patients were randomly divided into observation group oRcontrol group,with 36 cases in each group.AfteRoperation,both groups received conventional treatment,and moreover,the observation group was treated with Dantong Granules.The therapeutic effective rate,inflammation indicators 1 day before operation and the 7th day afteRoperation,liveRfunction indicators 1 day before operation,the 7th day and the 12th afteRoperation,and drainage volumes of bile from the 7th day to the 12th afteRoperation were compared between the two groups.Results There was no significant difference in the total therapeutic effective rate between the two groups( P >0.05).At the 7th day afteRoperation,the levels of WBC and C-reactive protein in both groups were loweRthan before,and the levels were loweRin observation group than in control group(all P <0.05).The levels of total bilirubin and ALT at the 7th day afteRoperation,and the direct bilirubin levels at the 7th and the 12th day afteRoperation were loweRin the control group than in the observation group(all P <0.05).The drainage volumes of bile from the 7th day to the 12th afteRoperation were more in the observation group than in the control group( P <0.05).Conclusion FoRpatients afteRcholecystectomy combined with choledochotomy and T-tube drainage,Dantong Granules is of obvious therapeutic effect,foRit can increase the drainage volume of bile,alleviate inflammation,and improve liveRfunction.
