摘要 目的:观察氨氯地平阿托伐他汀钙片治疗高血压合并冠心病的效果及其对血管内皮功能、血脂和炎症因子的影响。方法:回顾性分析2016年6月至2017年6月收治的96例高血压合并冠心病患者的临床资料,按照治疗方式分为对照组和观察组各48例。对照组给予硝苯地平缓释片治疗,观察组给予氨氯地平阿托伐他汀钙片治疗。比较两组治疗前后血脂水平、血管内皮功能及血清炎性因子水平的变化。结果:治疗后,两组的收缩压、舒张压、TC、TG、LDL-C及HDL-C水平均较治疗前有所改善,且观察组明显优于对照组,差异有统计学意义(P<0.05);两组血管内皮功能均有明显改善,且观察组NO水平明显高于对照组,ET-1水平明显低于对照组,差异有统计学意义(P<0.05);两组血清炎性因子均较治疗前降低,且观察组Hcy、CRP及TNF-α水平均明显低于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率与对照组比较差异无统计学意义(P>0.05)。结论:氨氯地平阿托伐他汀钙片治疗高血压合并冠心病,能显著降低患者的血压、血脂水平,改善血管内皮功能,缓解炎症情况,对于改善其预后具有重要的意义,且安全性较高。 Objective: To observe effects of Amlodipine besylate and Atorvastatin calcium tablets on hypertension complicated with coronary heart disease and their effects on vascular endothelial function, blood lipid and inflammatory factors. Methods: The clinical data of 96 patients with hypertension and coronary heart disease were retrospectively analyzed. According to the treatment methods, they were divided into control group and observation group with 48 cases in each. The control group was treated with Nifedipine sustained-release tablets, while the observation group was treated with Amlodipine besylate and Atorvastatin calcium tablets. The changes of blood lipid level, vascular endothelial function and serum inflammatory factors were compared between the two groups before and after the treatment. Results: After the treatment, the systolic blood pressure, diastolic blood pressure, and the levels of TC, TG, LDL-C and HDL-C of the two groups were improved compared with those before the treatment, those of the observation group were significantly better than those of the control group, and the differences were statistically significant ( P <0.05). The vascular endothelial function of the two groups was significantly improved. The NO level in the observation group was significantly higher than that in the control group, the level of ET-1 was significantly lower than that in the control group ( P <0.05), and the differences were statistically significant ( P <0.05). The levels of serum inflammatory factors in the two groups after the treatment were lower than those before the treatment. The levels of Hcy, CRP and TNF-α in the observation group were significantly lower than those in the control group, and the differences were statistically significant ( P <0.05). However, there was no difference in the incidence of adverse reactions between the two groups ( P >0.05). Conclusions: Amlodipine besylate and Atorvastatin calcium tablets for the patients with hypertension complicated with coronary heart disease can significantly reduce blood pressure and the blood lipid levels, improve the vascular endothelial function, relieve inflammation, and have an important significance for improving their prognosis with higher safety.
