摘要
目的探索胎龄与脐带组织中硒(Se)、钴(Co)、镉(Cd)、砷(As)、铅(Pb)、镍(Ni)、钼(Mo)、锌(Zn)、锰(Mn)元素含量的关系。方法采用电感耦合等离子体质谱(ICP-MS)检测2004年至2016年间因产前诊断引产或出生后诊断的166例神经管缺陷患儿脐带组织中Se、Co、Cd、As、Pb、Ni、Mo、Zn、Mn九种元素含量,元素含量采用中位值及四分位间距[M(P_(25),_(P75))]表示,采用有序多分类Logistic回归模型调整混杂因素后分析胎龄与元素含量的关系。结果脐带组织中Ni、Mo、Zn、Mn含量的中位值及四分位间距分别为28.99(20.10,49.76) ng/g、26.52(21.75,33.11) ng/g、52.69(44.79,61.78)μg/g、0.42(0.33,0.55)μg/g,且不同胎龄组的差异有统计学意义。以胎龄16~22周组为参照,Ni在38~44周组的OR值及95%CI为0.08(0.02-0.27);Mo在28~32周、38~44周组OR值及95%CI分别为2.91(1.06-8.02)、5.66(1.89-6.88);Zn在23~27周、33~37周组OR值及95%CI分别为0.32(0.13-0.76)、0.26(0.09-0.75);Mn在23~27周、28~32周、33~37周组OR值及95%CI分别为0.14(0.06-0.37)、0.28(0.10-0.79)和0.10(0.03-0.32)。脐带组织中Se、Co、Cd、As、Pb含量的中位值及四分位间距分别为0.56(0.50,0.64)μg/g、3.47(2.48,5.14) ng/g、1.33(0.57,3.51) ng/g、6.33(3.96,9.47) ng/g和27.32(18.37,50.34) ng/g,不同胎龄组间差异未见统计学意义。结论脐带组织中Zn、Mn、Mo、Ni量与胎龄有关,而Se、Co、Cd、As、Pb含量不随胎龄变化而改变。
Objective To explore the relationships between gestational age and the concentrations of selected elements in umbilical cord tissue, such as selenium (Se), cobalt (Co), cadmium (Cd), arsenic (As), lead (Pb), nickel (Ni), molybdenum (Mo), zinc (Zn) and manganese (Mn). Methods Inductively Coupled Plasma Mass Spectrometry (ICP-MS) was used to detect the concentrations of the above nine elements in dried umbilical cord tissue among 166 induced fetus or newborns with neural tube defects collected in 2004-2016. The median and interquartile range (IQR:P 25 ,P 75 ) were used to indicate the average element concentration. Multivariate ordinal logistic regression model was used to controlling potential confounders. Results The median and IQRs of Ni, Mo, Zn and Mn was 28.99 (20.10, 49.76) ng/g, and 26.52 (21.75, 33.11) ng/g, 52.69 (44.79, 61.78)μg/g, 0.42 (0.33, 0.55)μg/g respectively, with statistically significant difference among gestational age groups. Compare to element concentration of the umbilical cord tissues in 16-22 weeks, the OR value and the 95% CI of Ni in 38-44 weeks was 0.08 (0.02, 0.27);the corresponding OR and 95% CI of Mo in 28-32 weeks and 38-44 weeks was 2.91 (1.06, 8.02) and 5.66 (1.89-16.88), respectively;the OR value and 95% CI of Zn in 23-27 weeks and 33-37 weeks was 0.32 (0.13, 0.76) and 0.26 (0.09, 0.75);the OR value and 95% CI of Mn in 23-27 weeks, 28-32 weeks and 33-37 weeks was 0.14 (0.06, 0.37), 0.28 (0.10, 0.79) and 0.10 (0.03, 0.32). The median and IQRs of Se, Co, Cd, As, Pb was 0.56 (0.50, 0.64)μg/g, 3.47 (2.48, 5.14) ng/g, 1.33 (0.57, 3.51) ng/g, 6.33 (3.96, 9.47) ng/g and 27.32 (18.37, 50.34) ng/g, respectively;the difference of medians among groups of gestational age was not statistically significant. Conclusion The content of Zn, Mn, Mo and Ni was related to gestational age, while the content of Se, Co, Cd, As and Pb in umbilical cord did not change with gestational age.
