白内障术后干眼的原因分析及处理

Cause Analysis and Treatment of Dry Eye after Cataract Surgery

目的探讨白内障术后干眼的原因分析及处理及临床经验和应用价值。方法方便选取2015年9月—2018年4月该院收治的白内障术后干眼患者50例,均给予常规术后护理,对患者不同程度白内障术后干眼症进行分析,并对相应的症状做处理。结果护理后50例患者,仅有10例干眼症患者在术后7～10 d内得到缓解,20例患者出现了睑板腺功能障碍干眼症(χ2=9.028),15例患者患上了泪液不足型干眼症(χ2=7.876),5例患上了泪液动力学异常型干眼症(χ2=6.083)。差异有统计学意义(P<0.05)。其中对症处理睑板腺功能障碍干眼症10例经过15 d得到缓解,泪液不足型干眼症7例15～20 d得到缓解,泪液动力学异常型干眼症1例得到缓解用时30 d。结论白内障术后干眼的原因十分复杂,干眼症又分为许多种,要根据患者的状态进行处理,缓解干眼症为主要目的。手术前要仔细做检查,术前要做好准备,术后要按时用药,尽量减少白内障术后干眼症,要根据干眼症的具体类型做相应的治疗。

Objective To investigate the causes, treatment, clinical experience and application value of dry eye after cataract surgery. Methods 50 patients with dry eye after cataract surgery were convenient selected enrolled in our hospital from September 2015 to April 2018. All patients were given routine postoperative care. The patients were examined for dry eye syndrome after cataract surgery and the corresponding symptoms were treated. Results In 50 patients after nursing, only 10 patients with dry eye had a remission T value of 0.023 within 7-10 days after surgery. 20 patients had meibomian gland dysfunction dry eye syndrome(χ^2=9.028),15 patients with tear-deficient dry eye syndrome(χ^2=7.876), 5 patients with tear dynamic abnormal dry eye syndrome(χ^2=6.083). And the difference was statistically significant(P<0.05). Among them, 10 cases of symptomatic treatment of meibomian gland dysfunction dry eye were relieved after 15 days, 7 cases of tear-deficient dry eye were relieved in 15-20 days, and one case of tear dynamic abnormal dry eye was relieved for 30 days. Conclusion The cause of dry eye after cataract surgery is very complicated. Dry eye syndrome is divided into many kinds. It should be treated according to the patient's condition and relieve dry eye syndrome as the main purpose. Before the operation, you should carefully check, preoperative preparation, post-operative medication, to minimize dry eye after cataract surgery, according to the specific type of dry eye treatment.
