颅内血管因素三叉神经痛的MRI成像特点及诊断意义

MRI Imaging Features and Diagnostic Significance of Intracranial Vascular Factors Trigeminal Neuralgia

目的分析颅内血管因素的三叉神经痛MRI成像的特点与诊断意义。方法该次方便选取的70例三叉神经痛患者,入该院治疗的时间均是2017年6月—2018年6月,全部患者都采取MRI核磁共振诊断,评估诊断的义。结果该次所选70例三叉神经痛患者,有69例患者有异常血管压迫了三叉神经痛,有1例异常静脉压迫了三叉神经痛患者出现漏诊,9例有2支血管都对一侧的三叉神经产生压迫,1例一次的三叉神经受到三支血管压迫。43例为动脉压迫,其中,小脑上的动脉19例、小脑前的下动脉10例、迷路动脉3例、小脑后的下动脉5例、椎动脉3例、基底动脉的延长扩张2例、1例患者永存的三叉动脉。23例患者是静脉压迫,桥静脉17例、脑桥三叉的静脉3例,中脑外侧的静脉1例,漏诊小脑中脚的静脉2例。血管畸形3例,动静脉的畸形2例,海绵状血管的畸形1例。该次所选70例患者中没有发现动静脉瘘、动脉瘤以及颈动脉的海绵窦瘘血管等因素的三叉神经痛。结论颅内血管因素的三叉神经痛MRI成像能够清楚显示患者各种动脉压迫情况,为临床治疗提供参考。

Objective To analyze the characteristics and diagnostic significance of MRI imaging of trigeminal neuralgia in intracranial vascular factors. Methods The time of treatment of 70 patients with trigeminal neuralgia in our hospital was convenient selected from June 2017 to June 2018. All patients underwent MRI MRI diagnosis to evaluate the meaning of diagnosis. Results In this selected 70 patients with trigeminal neuralgia, 69 patients had abnormal vascular compression of trigeminal neuralgia, 1 patient with abnormal venous compression of trigeminal neuralgia was missed, and 9 patients had 2 vessels on one side trigeminal nerve was compressed, and one case of the trigeminal nerve is compressed by three vessels. 43 cases were arterial compression, including 19 cases of cerebellar artery, 10 cases of anterior cerebellar artery, 3 cases of labyrinth artery, 5 cases of posterior cerebellar artery, 3 cases of vertebral artery, and 2 cases of basilar artery extension. The permanent trigeminal artery of the patient. 23 patients were treated with venous compression, 17 cases of bridge vein, 3 cases of cerebral trigeminal vein, 1 case of lateral midbrain vein, and 2 cases of missed cerebellar midfoot, 3 cases of vascular malformation, 2 cases of arteriovenous malformation, and 1 case of spongy vascular abnormality. Trigeminal neuralgia such as arteriovenous fistula, aneurysm, and cavernous sinus vessels of the carotid artery were not found in the 70 patients selected. Conclusion MRI imaging of trigeminal neuralgia with intracranial vascular factors can clearly show the various arterial compressions of patients and provide reference for clinical treatment.
