腹针治疗退变性腰椎间盘突出症的早中期临床观察

Early and Mid-term Observation of Abdominal Acupuncture in the Treatment of Degenerative Lumbar Disc Herniation

目的探讨腹针治疗因退变性腰椎间盘突出引起腰腿痛的早、中期疗效及生存质量的改善。方法选取我院2016年1月—2017年7月腰椎间盘突出症的患者110例随机分为腹针组和体针组,随访12月;统计分析2组治疗前后神经功能JOA29分法、视觉模拟疼痛评分(VAS)、Oswestry功能障碍指数量表(ODI)评分、生存质量SF-36评分。结果 110例患者随机分为腹针组52例、常规体针组58例,所有参与者随访12月;2组最终纳入96例,共失访6例,失访率5.5%。经统计学分析2组治疗1个疗程后、治疗后6月在VAS评分、JOA评分、神经功能改善率、ODI评分、SF-36评分方面均有统计学意义(P<0.05),腹针治疗组优于常规体针治疗;而在治疗后12月疼痛VAS评分、神经功能JOA评分、神经功能改善率、ODI评分、生存质量SF-36评分方面均无统计学意义(P>0.05)。结论腹针治疗退变性腰椎间盘突出症引起的腰腿痛在早期不仅能缓解患者疼痛而且能有效的改善患者生存质量,是中医治疗的一种有效方法。

Objective To investigate the early and mid-term effects of abdominal acupuncture on lumbago and leg pain caused by degenerative lumbar disc herniation and the improvement of the quality of life.Methods 110 patients with lumbar disc herniation in our hospital from January 2016 to July 2017 were randomly divided into the abdominal acupuncture group and body acupuncture group,and all participants were followed up for 12 months.Analysis of the before and after treatment was made,the two groups had 29 JOA scores,visual analogue pain score,(ODI)score of(VAS),Oswestry dysfunction index and SF-36 score of quality of life.Results 96 cases were included in the two groups,6 cases were lost visit,and the rate of missing visit was 5.5%.After a course of treatment in the two groups,the scores of pain VAS,nerve function JOA,neural function improvement were evaluated at 6 months after treatment.There was the significant difference in SF-36 score of quality of life(P<0.05),abdominal acupuncture treatment group was superior to routine body acupuncture treatment group,but at 12 months after treatment,VAS score,JOA score,nerve function improvement rate and neurological function improvement rate were evaluated.There was no significant difference in SF-36 score(P>0.05).Conclusion Abdominal acupuncture treatment of low back pain caused by the degenerative lumbar disc herniation in the early stage can not only relieve the pain of patients but also improve the quality of life of patients effectively.It is an effective method of traditional Chinese medicine to treat the low back and leg pain caused by degenerative lumbar disc herniation.
