摘要
目的:探讨双能量CT血管造影(DECTA)及单能量CT血管造影(SECTA)在颈动脉体瘤(CBT)诊断中的应用价值。方法:选取30例CBT患者,收集其DECTA检查资料;同时选取30名行DECTA、单能量CT血管造影(SECTA)检查的头颈部动脉无病变者,比较两种方法检查图像质量及放射剂量。结果:30例CBT患者行手术切除并经病理证实,DECTA检查与术中所示完全吻合;30例CBT患者均位于颈动脉分叉处,呈现典型"抱球状"或"高脚杯"状改变;DECTA检查中的最大密度投影(MIP)和容积再现(VRT)可清晰显示瘤体与颈动脉的关系;DECTA检查颈内静脉CT值为(330.41±60.87)HU,明显高于SECTA检查,其差异有统计学意义(t=-9.852,P<0.05);DECTA竖脊肌和冈上肌CT值噪声分别为(10.51±3.41)和(20.14±3.54),明显低于SECTA,其差异有统计学意义(t=-10.698,t=-12.221;P<0.05);DECTA检查的放射剂量为(484.46±54.02)m Gy·cm,明显低于SECTA,其差异有统计学意义(t=-8.631,P<0.05)。结论:DECTA在CBT诊断中有较高的应用价值,具有检查方便、无创等优点;与SECTA检查相比,DECTA检查辐射剂量小,而图像质量总体相当。
Objective: To investigate the application values of dual energy CT angiography(DECTA) and single energy CT angiography(SECTA) in the diagnosis of carotid body tumor(CBT). Methods: 30 patients with CBT were selected, and their DECTA examination data were collected. At the same time, 30 patients without lesion on head and neck artery who were respectively examined by DECTA and single energy CT angiography(SECTA) were selected. And the image quality and the dose of radiation of two methods were further compared. Results: After 30 patients underwent surgical resection and were confirmed by pathology, the results of DECTA examination were fully identical with the results of operation. And the CBT of 30 patients located in the carotid bifurcation, and they showed typical changes of "Hold ball shape" or "goblet shape". Maximum intensity projection(MIP) and volume rendering technique(VRT) of DECTA can clearly show the relationship between the tumor body and the carotid artery. The CT value(330.41±60.87HU) of internal jugular vein of DECTA was significantly higher than that of SECTA(t=-9.852, P<0.05). And the CT value noise of the erector spinae and supraspinatus of DECTA were(10.51±3.41) and(20.14±3.54), respectively, and they were significantly lower than that of SECTA(t=-10.698, t=-12.221, P<0.05). Besides, the radiation dose of DECTA was(484.46±54.02) mGy·cm, which was significantly lower than that of SECTA(t=-8.631, P<0.05). Conclusion: DECTA has higher application value in the diagnosis of CBT, which has the advantages includes convenient examination, noninvasive, etc.. Compared with the SECTA examination, the radiation dose of the DECTA is small, while the image qualities of them are totally equality.
