超声清创机清洗肛周脓肿术后创面的清洗效果及疗效观察

Observation on cleaning and curative effect of ultrasonic debridement machine in cleaning the postoperative surface of wound of perianal abscess

目的:探究肛周脓肿术后使用超声清创机进行清洗的效果。方法:选取77例肛周脓肿术患者,运用随机抽签法将其分为对照组(38例)与观察组(39例),对照组术后给予常规处理,观察组术后使用常规敷料包扎换药联合超声清创机清洗肛周,采用视觉模拟评分法(VAS)对术后伤口疼痛程度进行分析;比较两组术后伤口细菌清除率、创面愈合时间、感染控制时间及创面清洁效果。结果:术后第1d、3d、5d和9d时评估创口疼痛,观察组VAS评分均明显低于对照组,差异有统计学意义(t=8.031,t=12.557,t=12.857,t=12.902;P<0.05);观察组清洁度良占比87.18%,明显高与对照组的65.79%,其差异有统计学意义(x^2=12.719,P<0.05);且观察组创面细菌清除率高于对照组,而住院时间低于对照组,其差异有统计学意义(t=56.441,t=13.348;P<0.05);观察组治疗总有效率明显高于对照组,其差异有统计学意义(x^2=11.115,P<0.05);观察组患者创面愈合、感染控制时间均低于对照组,其差异有统计学意义(t=24.762,t=18.190;P<0.05);且术后脓肿复发率低于对照组,其差异有统计学意义(x^2=19.566,P<0.05)。结论:采用超声清创机在肛周脓肿术后清洗创面,可有效清除创面细菌,提高清洗效果,减轻创口疼痛程度,促进伤口快速修复。

Objective: To investigate the effect of ultrasonic debridement machine for postoperative cleaning of perianal abscess. Methods: 77 patients with perianal abscess were enrolled in this study. They were divided into control group(38 cases) and observation group(39 cases) by random sampling. The control group was implemented routine treatment post operation and the observation group was implemented routine dressing combined with ultrasonic cleaning machine to clean the crissum. And the visual analogue scale(VAS) was adopted to analyze the pain level of postoperative wound. And the postoperative clearance rate of bacteria, wound healing time, controlled time of infection and cleaning effect of wound between two groups were compared. Results: The VAS score of the observation group on the 1st, 3rd, 5th and 9th day after operation were significantly lower than those of control group(t=8.031, t=12.557, t=12.857, t=12.902, P<0.05), respectively. And the rate of good cleanliness of observation group(87.18%) was significantly higher than that(65.79%) of the control group(x^2=12.719, P<0.05). And clearance rate of bacteria on wound of observation group was significantly higher than that of control group while the length stay in hospital of observation group was significantly lower than that of control group(t=56.441, t=13.348, P<0.05). The total effective rate of observation group was significantly higher than that of control group(x^2=11.115, P<0.05). And the wound healing time and controlled time of infection of observation group were significantly lower than those of control group(t=24.762, t=18.190, P<0.05), respectively. Besides, the recurrence rate of abscess post operation of observation group was significantly lower than that of control group(x^2=19.566, P<0.05). Conclusion: Adopted ultrasonic debridement machine to clean wounds after perianal abscess surgery can effectively remove bacteria of wound, and improve cleaning effect, alleviate pain level of wound, and promote the rapid repair of wounds.
