摘要 目的:探讨甲钴胺联合甘精胰岛素治疗肥胖型2型糖尿病的疗效及对患者血糖水平的影响。方法:回顾性选取2017年1月至2018年4月湖北省谷城县人民医院收治的肥胖型2型糖尿病患者100例,根据治疗方案的不同分为观察组与对照组,每组50例。对照组患者仅给予甘精胰岛素治疗,观察组患者在对照组的基础上加用甲钴胺。观察两组患者的临床疗效、治疗前后空腹血糖、餐后2 h血糖、糖化血红蛋白、体质量指数及治疗期间不良反应发生情况。结果:观察组患者的总有效率为96.0%(48/50),明显高于对照组的82.0%(41/50),差异有统计学意义(P<0.05);治疗后,观察组患者空腹血糖、餐后2 h血糖、糖化血红蛋白及体质量指数水平明显低于对照组,差异均有统计学意义(P<0.05);两组患者治疗期间均无明显不良反应发生。结论:甲钴胺联合甘精胰岛素治疗肥胖型2型糖尿病的疗效较好,能够有效控制患者的血糖水平,降低体质量指数。 OBJECTIVE: To probe into the efficacy of mecobalamin combined with insulin glargine in treatment of obesity type Ⅱ diabetes mellitus and its effects on blood glucose levels. METHODS: 100 patients with obesity type Ⅱ diabetes mellitus admitted into Hubei Gucheng County People’s Hospital from Jan. 2017 to Apr. 2018 were retrospectively selected and divided into observation group and control group according to different therapeutic regimens, with 50 cases in each. The control group was treated with insulin glargine, while the observation group was given mecobalamin combined with insulin glargine. The clinical efficacy, fasting blood glucose(FBG), 2 h postprandial blood sugar, glycosylated hemoglobin, BMI and incidences of adverse drug reactions of two groups were observed. RESULTS: The total effective rate of observation group was 96.0%(48/50), which was significantly higher than that of the control group(82.0%, 41/50), with statistically significant difference(P<0.05);after treatment, the FBG, 2 hPBG, glycosylated hemoglobin and BMI of observation group were significantly lower than those of the control group, with statistically significant differences(P<0.05);there were no significant adverse drug reactions in both groups during treatment. CONCLUSIONS: The efficacy of mecobalamin combined with insulin glargine in treatment of obesity type Ⅱ diabetes mellitus, which can effectively control patients’ blood glucose levels and reduce BMI.
