坤泰胶囊致重度药物性肝损伤一例

Analysis on One Case of Severe Drug-Induced Liver Injury Caused by Kuntai Capsules

1例33岁女性患者,口服坤泰胶囊17 d后,出现食欲减退、呕吐,同时伴有眼黄、尿黄及皮肤黄染;继续服用3 d后,纳差加重,肝功能指标异常,B超检查提示胆囊炎;立即停用坤泰胶囊,予以保肝、降酶和退黄治疗,后肝功能指标水平明显改善,病情好转,予以出院。考虑为坤泰胶囊引起的重度药物性肝损伤,可能与坤泰胶囊中黄芩和黄连成分有关。临床医师应严格按照《中成药临床应用指导原则》使用中成药,有药物过敏史者及特殊人群更应慎用。

A 33-year-old female patient,after 17 days of oral administration of Kunming capsules,the patient developed anorexia and vomiting,accompanied with eyes yellow,yellow urine and xanthochromia;after another 3 days of administration,the anorexia had been aggravated,with abnormal indicators of liver function,and B-ultrasound indicated cholecystitis;the patient was given drug withdrawal of Kuntai capsules immediately,treated with liver protection,enzyme reduction and removing jaundice,after the liver function indices significant improved,condition improved,then the patient was agreed to discharged.It was considered the severe drug-induced liver injury was caused by Kuntai capsules,which might be related to the components of astragalus and coptidis.Clinicians should strictly use Chinese medicines in accordance with the Guiding Principles for Clinical Application of Chinese Patent Medicines,those patients with history of drug allergies and special populations need more caution.
