重症肺炎需呼吸机支持患者的液体负平衡与预后的关系

Relationship between fluid negative balance and prognosis in severe pneumonia patients requiring ventilator support

目的探讨重症肺炎需呼吸机支持患者的液体负平衡与预后的关系。方法研究时间为2015年2月到2018年1月,选择在本院MICU诊治的重症肺炎患者60例作为研究对象,根据随机抽签原则分为观察组与对照组,各30例。所有患者都给予呼吸机支持治疗,观察组给予液体负平衡干预,对照组给予液体正平衡干预,记录与观察两组预后。结果观察组的液体日均入量与日均出量显著少于对照组,差异有统计学意义(P<0.05),脱机当天液体平衡量与对照组对比也有显著差异,有统计学意义(P<0.05)。观察组的呼吸机通气时间少于对照组,2周内撤机成功率高于对照组(96.7%和73.3%),对比差异有统计学意义(P<0.05)。结论重症肺炎需呼吸机支持患者的液体负平衡能提高撤机成功率,缩短呼吸机支持时间。

Objective To explore the relationship between fluid negative balance and prognosis in severe pneumonia patients requiring ventilator support. Methods From February 2015 to January 2018, 60 patients with severe pneumonia treated in our hospital MICU were selected as the research subjects, all the patients were divided into the observation group and the control group of 30 patients in each groups accorded to the random draw principle. All the patients were given the ventilator support treatment, the observation group were given liquid negative balance intervention, and the control group were given positive liquid balance. The prognosis of the two groups was recorded and observed. Results The daily average and daily output of liquid in the observation group were significantly less than that of the control group, the difference was statistically significant (P< 0.05), and there was significant difference compared between the two groups of the day of liquid balance offline, statistically significant (P<0.05). The time of ventilator ventilation in the observation group were less than that in the control group, the success rate of weaning in 2 weeks were higher than that in the control group (96.7% and 73.3%), and compared the difference between the two groups were statistically significantly (P<0.05). Conclusion The negative balance of patients with severe pneumonia needs ventilator support, which can improve the success rate of weaning and shorten the support time of respirator.
