前列腺增生症患者病耻感及应对方式的相关性

Correlation between stigma and coping styles in patients with prostatic hyperplasia

目的了解前列腺增生症患者病耻感和应对方式现状,探讨二者的相关性。方法采用一般资料问卷、社会影响量表(social impact scale, SIS)和简易应对方式问卷(simlified coping style questionnaire, SCSQ)对151例前列腺增生症患者进行调查。结果患者SIS总分为(57.19±3.07)分,其中内在羞耻、社会隔离均分为(3.01±0.21)分、(2.51±0.69)分;不同文化程度、医疗费用支付方式、疾病相关知识知晓程度、症状严重程度的患者SIS得分比较,差异有统计学意义(均P<0.05);与国内常模比较,患者消极应对均分较高,积极应对均分较低(均P <0.001)。SIS总分以及各维度得分与消极应对得分呈正相关关系(均P<0.01);SIS总分以及内在羞耻维度、社会隔离维度得分与积极应对得分呈负相关关系(均P<0.01)。结论前列腺增生症患者病耻感处于中等偏高水平,且面对疾病时较为消极。应采取积极有效的应对方式增强患者应对疾病的信心,以降低患者病耻感,从而改善患者生活质量。

Objective To study the status of stigma and coping styles in patients with prostatic hyperplasia and explore reasonable and effective nursing intervention measures.Method One hundred and fifty-one patients with prostatic hyperplasia were enrolled in the investigation preoperatively by general data questionnaire,social impact scale(SIS)and simple coping style questionnaire(SCSQ).Results The total score of SIS was(57.19±3.07),the inner shame and social isolation were the highest with strong stigma.There were differences in stigma among patients with different education levels,payment methods of medical expenses,disease-related knowledge and severity of symptoms(P<0.05).Compared with domestic model,negative coping scores were significantly higher,and positive coping scores were significantly lower(all P<0.001).There was a positive correlation between negative coping style and total score of stigma and score of all dimensions,and positive coping style was negatively correlated with total score of stigma,internal shame dimension and social isolation dimension(all P<0.01).Conclusions There is a strong sense of stigma in patients with prostatic hyperplasia,and it is relatively passive about the disease.The nursing staff should help them alleviate the stigma by improving patients'coping style,enhance their confidence in coping with diseases and improve patients'quality of life.
