Effect Evaluation of Risk Management on High-risk Drugs in Geriatric Department

目的 探讨质量管理对老年科高危药品的管理成效。方法 回顾性分析选择2016年1—12月与2017年1—12月高危药的管理制度执行实施后的同期环比。质量控制小组按年份进行分组,2016年1—12月设为对照组,2017年1—12月设为观察组,按医院对高危药质控管理规范标准要求进行48次督查,督查内容:高危药的制度执行有效性、警示标示的规范性、专柜专放、是否规范操作、药物配伍禁忌等方面。高危药的管理缺陷、责任护士对高危药的掌握采用t检验;患者对责任护士满意度采用百分比统计。结果 医院对高危药进行统一管理后,提高护士责任心,增加与药剂科沟通,落实巡视及时与患者沟通,使患者满意度大幅度提高。在高危药品管理中,记录出入量的登记数目P<0.101,巡视规范执行P<0.101,专人专柜放置及保管P<0.101。责任护士对高危药的知识掌握明显提高,观察组高于对照组,t=4.243,P<0.01。同时患者对责任护士满意度均提高。结论 医院统一对高危药品管理及对护士规范培训,使护士在临床中按高危药品规范执行,并将高危药使用风险降低到最小程度。

Objective To explore the effect of quality management on the management of high-risk drugs in Geriatric Department.Methods A retrospective analysis was conducted to compare the same period between January to December in 2016,and January to December in 2017 before and after the implementation of high-risk drugs management system.The quality control team was grouped by year,with January to December in 2016 as the control group,and January to December in 2017 as the observation group.According to the hospital standard management requirements on high-risk drug quality control,48 inspections were carried out,which were:the effectiveness of the system implementation of high-risk drugs,the normative of warning signs,special counters,standard operation,drug compatibility taboos and other aspects.The management defects of high-risk drugs and the mastery of high-risk drugs by responsible nurses were analyzed by T-test,and the patients'satisfaction with responsible nurses was counted as percentage.Results After unified management of high-risk drugs,the hospital strengthened the sense of responsibility of nurses,increased communication with the pharmacy department,improved the implementation of timely communication with patients,so that patients'satisfaction had greatly improved.In the management of high-risk drugs,record the number of registrations of access P<0.101,and inspection norms to implement P<0.101,and dedicated counters to place and keep P<0.101.The knowledge of the responsible nurses on the high-risk drugs improved obviously,and the observation group was higher than that in the control group,t=4.243,P<0.01.At the same time,the satisfaction of the patients with the responsible nurses increased.Conclusion The hospital unifies the management of high-risk drugs and the standard training for nurses,so that nurses in clinical could practice according to the high-risk drug norms,and reduce the risk of high-risk drug usage to a minimum.
