助产责任制护理联合产时体位管理对产妇分娩方式与分娩结局的影响

Effect of Midwifery Responsibility System Nursing Combined with Postural Management on Maternal Delivery Mode and Outcome

目的:探讨助产责任制护理联合产时体位管理对产妇分娩方式与分娩结局的影响。方法:在医院2014年12月～2015年11月期间诊治的产妇102中抽取102例作为研究对象,并依照随机抽签原则分两组,对照组(n=51)应用助产轮班制护理,研究组(n=51)则采取助产责任制护理联合产时体位管理,就两组产妇的分娩结局以及分娩方式进行统计学分析。结果:(1)研究组产妇的自然分娩率较高,剖宫产率较低,与对照组相比均有统计学差异(P<0.05);同时,研究组产妇的阴道助产率低于对照组,但其比较不具备统计学差异(P>0.05);(2)研究组产程异常、产后出血、下肢麻木、新生儿窒息、酸中毒发生率均低于对照组,在产后出血、下肢麻木颌酸中毒发生率比较上有统计学差异(P<0.05),其余无统计学差异(P>0.05)。结论:助产责任制护理联合产时体位管理用于产妇分娩中,可改善其分娩方式、分娩结局,有推广意义。

Objective:To explore the effect of midwifery responsibility system nursing combined with postural management on maternal delivery mode and outcome.Methods:102 parturients who were treated by the hospital from December 2014 to November 2015 were selected as research objects,and divided into two groups according to the principle of random lottery.The control group(n=51)applied the nursing of midwifery shift system,the research group(n=51)adopted the nursing of midwifery responsibility system combined with postural management,and conducted statistical analysis on the outcome and delivery mode of the two groups of women.Results:(1)The natural delivery rate and cesarean section rate of women in the study group were higher than those in the control group(P<0.05).At the same time,the vaginal midwifery rate of the study group was lower than that of the control group,but there was no statistical difference(P>0.05).(2)The incidence of abnormal labor process,postpartum hemorrhage,numbness of lower limbs,neonatal asphyxia and acidosis in the research group was lower than that in the control group.There were statistically significant differences in the incidence of postpartum hemorrhage and numbness of lower limbs and jaw acidosis(P<0.05),while there were no statistically significant differences in the rest(P>0.05).Conclusion:Midwifery responsibility system nursing combined with postural management for maternal delivery,can improve its delivery mode,delivery outcome,has the promotion significance.
