摘要
目的分析超声心动图指标、血清脑钠素N端前体肽(NT-proBNP)及同型半胱氨酸(Hcy)与慢性充血性心力衰竭(CHF)病情的关系。方法回顾性分析200例CHF患者(观察组)与同期健康体检者50例(对照组)的彩色多普勒超声心动图、血清脑钠素N端前体肽(NT-proBNP)及同型半胱氨酸(Hcy)检测结果,比较两组心功能[左室射血分数(LVEF)、左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)]、血清NT-proBNP及Hcy水平,以及不同NYHA分级CHF患者上述指标的变化,分析心功能与血清NT-proBNP及Hcy水平的相关性。结果观察组LVEF低于对照组,而LVEDV、LVESV及血清NT-proBNP、Hcy水平高于对照组(P <0.05);CHF患者中,随NYHA分级增加,LVEDV、LVESV和血清NT-proBNP、Hcy水平上升,而LVEF降低(P <0.05);CHF患者LVEF与血清NT-proBNP、Hcy水平呈负相关,LVEDV、LVESV与血清NT-proBNP、Hcy水平均呈正相关(P <0.05)。结论超声心动图、血清NT-proBNP、Hcy水平与CHF患者病情严重程度有关,且各指标间存在一定相关性,可用于CHF的诊断和病情评估。
Objective To analyze the relationship of echocardiographic parameters,serum N-ter^ninal precursor brain natriuretic peptide(NT-proBNP)and homocysteine(Hcy)with chronic congestive heart failure(CHF).MeAods The results of color Doppler echocardiography and NT-proBNP and Hcy levels in serum in 200 patients with CHF(“observation group”)and 50 healthy subjects("control group")were retrospectively analyzed.The left ventricular function[including left ventricular ejection fraction(LVEF),left ventricular end-diastolic volume(LVEDV)and left ventricular end-systolic volume(LVESV)1,the NT-proBNP and Hcy levels in serum,as well as the changes of above-mentioned indicators in CHF patients with different NYHA grades were compared between the two groups,and the correlation of the cardiac function with NT-proBNP and Hcy levels in serum was analyzed.Resulte The LVEF in the observation group were lower than those in the control group,but the LVEDV,LVESV,NT-proBNP and Hcy levels in serum were higher than those in the control group(P<0.05).In the CHF patients,with the increase of NYHA classification,the LVEDV,LVESV,NT-proBNP and Hcy levels in serum increased,while the LVEF decreased(P<0.05);the LVEF of the patients were negatively correlated with the NT-proBNP and Hcy levels in serum,while the LVEDV and LVESV were positively correlated with NT-proBNP and Hcy levels in serum(P<0.05).Conclusion Echocardiography,NT-proBNP and Hcy levels in serum are related to the severity of CHF,and there is a certain correlation among the indicators,which can be used for the diagnosis and evaluation of CHF.
