PRECEDE模式护理改善老年慢性乙型肝炎患者遵医行为及生活质量的效果研究

Effect of PRECEDE mode nursing on clinical effects, compliance behavior and quality of life in elderly patients with chronic hepatitis B

目的探究PRECEDE模式护理应用于老年慢性乙型肝炎患者中对临床效果、遵医行为及生活质量的影响。方法选取2017年5月至2018年5月华中科技大学同济医学院附属协和医院感染科收治的98例老年慢性乙型肝炎患者临床资料进行分析,将行常规护理的49例患者作为对照组,行PRECEDE模式护理的49例患者作为研究组,比较两组患者健康知识知晓性、遵医行为、生活质量。结果研究组患者的疾病成因、自我管理认知等健康知识知晓性评分为17.28±1.59、17.54±1.59,均高于对照组的12.35±1.64、12.52±1.63(均P<0.05);研究组的按医嘱取药、定期复查和检查等遵医行为评分也均高于对照组(23.48±1.33、23.59±1.37比19.35±1.28、19.58±1.53)(P<0.05);研究组的社会功能、物质功能等生活质量评分与对照组相比均较高(82.37±3.21、82.54±3.16比67.57±3.12、67.59±3.28)(P<0.05)。结论老年慢性乙型肝炎患者经PRECEDE模式护理后可有效提高健康知识知晓性、遵医行为,改善生活质量。

Objective To investigate the effects of PRECEDE model nursing on clinical outcomes,compliance behavior and quality of life in elderly patients with chronic hepatitis B.Methods The clinical data of 98 elderly patients with chronic hepatitis B admitted into the Union Hospital affiliated to Tongji Medical College of Huazhong University of Science and Technology from May 2017 to May 2018 were analyzed.The routine caregivers were observed as the control group(49 cases),and the caregivers treated with PRECEDE model were enrolled into the study group(49 cases).The health knowledge,compliance behavior and quality of life between the two groups were compared.Results The scores of health knowledge including pathogenesis(17.28±1.59)and self-management(17.54±1.59)in study group were higher than those of control group which was 12.35±1.64 and 12.52±1.63,respectively.The scores of compliance behaviors including following the doctors advice(23.48±1.33)and regular reexamination(23.59±1.37)in study group were higher than those of control group which was 19.35±1.28 and 19.58±1.53,respectively.The scores of life quality including social function(82.37±3.21)and substance function(substance function)in study group were higher than those of control group which was 67.57±3.12 and 67.59±3.28,respectively(P<0.05).Conclusions The health knowledge,compliance behavior and quality of life can be improved by the using of PRECEDE mode nursing.
