加味益气降浊汤联合丹红注射液对痰阻心脉证冠心病不稳定型心绞痛患者心电图变化以及血清TIMP-1、sICAM-1水平变化的影响

Effect of Jiawei Yiqi Jiangzhuo Decoction with Danhong Injection on Electrocardiogram and Changes of Serum TIMP-1 and sICAM-1 Levels in Patients with Unstable Angina Pectoris

目的:探讨加味益气降浊汤联合丹红注射液对痰阻心脉证冠心病不稳定型心绞痛患者心电图(echocardiography,ECG)变化以及血清基质金属蛋白酶组织抑制物-1(TIMP-1)、可溶性细胞间粘附因子-1(sICAM-1)水平变化的影响。方法:选择2016年9月至2018年3月于我院就诊的84例痰阻心脉证冠心病不稳定型心绞痛患者随机分为观察组(42例)和对照组(42例)。观察组患者给予加味益气降浊汤联合丹红注射液治疗,对照组给予单独丹红注射液治疗。比较两组患者的疗效、心绞痛评分量表(GRACE)评分、中医主要临床症状评分、心电图变化、血清TIMP-1和sICAM-1水平变化以及不良反应。结果:观察组治疗后的心绞痛改善总有效率(90.48%)高于对照组(64.29%)(P<0.05);两组患者治疗后的GRACE评分均下降,而且观察组低于对照组(P<0.05);两组患者治疗后的中医临床症状总评分均下降,而且观察组低于对照组(P<0.05);观察组治疗后的心电图改善总有效率(88.10%)比对照组(66.67%)高(P<0.05);两组患者治疗后的血清TIMP-1水平上升,sICAM-1水平下降,且观察组比对照组改善更佳(P<0.05)。结论:加味益气降浊汤联合丹红注射液治疗痰阻心脉证冠心病不稳定型心绞痛可提高疗效,缓解临床症状,改善心电图,调节血清TIMP-1和sICAM-1水平,且安全性高。

Objective : To investigate the effects of Jiawei Yiqi Jiangzhuo decoction combined with Danhong injection on echocardiography (ECG) changes and serum TIMP-1 and sICAM-1 levels in patients with unstable angina pectoris. Methods : 84 patients with unstable angina pectoris of coronary heart disease who were admitted to our hospital from September 2016 to March 2018 were randomly divided into the observation group (42 cases) and the control group (42 cases). Patients in the observation group were treated with Jiawei Yiqi Jiangzhuo decoction combined with Danhong injection, and the control group was treated with Danhong injection alone. The efficacy, angina score scale (GRACE), TCM main clinical symptom score, ECG changes, serum TIMP-1 and sICAM-1 levels, and adverse reactions were compared between the two groups. Results : The total effective rate of improvement of angina after treatment in the observation group(90.48%) was higher than that of the control group (64.29%)( P< 0.05). The GRACE scores of the two groups were decreased after treatment, and the observation group was lower than the control group ( P< 0.05). The total scores of clinical symptoms of TCM after treatment decreased in both groups, and the observation group was lower than the control group ( P< 0.05). The total effective rate of improvement of electrocardiogram after treatment in the observation group (88.10%) was higher than that of the control group (66.67%)( P< 0.05). Serum TIMP-1 levels increased after treatment, and sICAM-1 levels decreased, and the difference between the observation group and the control group was statistically significant( P< 0.05). Conclusion : Jiawei Yiqi Jiangzhuo decoction combined with Danhong injection in the treatment of unstable angina pectoris of coronary heart disease can improve the curative effect, relieve clinical symptoms, improve electrocardiogram, regulate serum TIMP-1 and sICAM-1 levels, and with high safety.
