炙甘草汤治疗冠心病室性早搏的Meta分析

A Mate analysis of treating premature ventricular contractions after coronary heart disease with the Zhigancao decoction

目的:应用Meta分析评价炙甘草汤干预冠心病室性早搏的疗效和安全性。方法:依照循证医学的要求,全面检索中国知网和万方数据库中自建库至2018年4月发表的炙甘草汤治疗冠心病室性早搏的随机对照临床研究相关文献,把符合纳入标准的7篇文献,共660例患者作为Meta分析的对象,选择室性早搏改善情况作为效应指标。应用Review Manager 5.3专用Meta软件进行统计分析。结果:7篇文献的OR=0.34,95%CI为[0.22,0.50],整体效果检验Z=5.23,P <0.00001,表明炙甘草汤治疗冠心病室性早搏与单纯应用常规药物治疗对比,差异的统计学意义。结论:炙甘草汤治疗冠心病室性早搏安全有效,但由于所纳入研究的文献经改良Jadad评分显示均属于低质量文献,故证据质量相对较差,仍需大样本、严格设计的随机对照研究进行进一步的验证。

Objective: To evaluate the efficacy and safety of the Zhigancao decoction (炙甘草汤) on premature ventricular contractions (PVC) after coronary heart disease (CHD) by Meta analysis. Methods: According to the requirements of evidence-based medicine, the relevant literature on the randomized controlled clinical study on the treatment of PVC after CHD , published in the CNKI and Wanfang database before April 2018, was retrieved in an all-round way. 7 articles in accordance with the standard were collected and 660 patients were selected as the object of Meta analysis. The improvement of ventricular premature contractions was selected as the index of effect. Review Manager 5.3 special meta software was used for statistical analysis. Results: OR=0.34, 95%CI of 7 literature was [0.22, 0.50];and Z =5.23, P < 0.000 01. It showed that there was a difference in the contrast between the treatment of PVC after CHD and the simple use of conventional medicine. Conclusion: The treatment of the Zhigancao decoction is safe and effective for PVC after CHD. However , the improved Jadad score shows that the quality of the evidence is low quality literature, so the quality of the evidence is relatively poor, and it is still necessary to further verify the large sample and strictly designed randomized controlled study.
