桃红四物汤辨证加减配合中医骨伤手法治疗腰椎间盘突出症(气滞血瘀证)的疗效分析

Efficacy of the Taohong Siwu decoction and TCM bone manipulation on lumbar disc herniation of the Qizhi Xueyu symptom

目的:分析在腰椎间盘突出症(气滞血瘀证)的治疗中应用桃红四物汤加减配合中医骨伤手法的疗效。方法:选取2017年1-12月医院收治的102例气滞血瘀型腰椎间盘突出症患者,以随机的方式将患者分为两组,对照组(51例)予以常规西药治疗,观察组(51例)在对照组基础上应用桃红四物汤辨证加减配合中医骨伤手法,对比两组治疗前后的炎症因子水平、症状评分变化和腰椎功能改善情况,并对比两组的治疗效果。结果:观察组与对照组在治疗前的血清肿瘤坏死因子(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)水平比较差异无统计学意义(P> 0.05),治疗后两组的血清TNF-α、hs-CRP、IL-6水平均降低(P <0.05),两组间比较观察组更低(P <0.05);两组治疗前的症状积分与腰椎功能日本骨科协会评估治疗分数(JOA)比较差异无统计学意义(P> 0.05);治疗后两组的症状评分均降低(P <0.05),JOA评分均提高(P <0.05),观察组的症状评分低于对照组(P <0.05),JOA评分高于对照组(P <0.05);两组的治疗有效率比较,观察组高于对照组(P <0.05)。结论:在气滞血瘀型腰椎间盘突出症的治疗中应用桃红四物汤辨证加减配合中医骨伤手法,可更好地降低炎症因子水平,促进患者症状的恢复,改善腰椎功能,并能提高治疗效果,值得临床推广应用。

Objective: To analyze the efficacy of the Taohong Siwu decoction (桃红四物汤) and TCM bone manipulation on lumbar disc herniation of the Qizhi Xueyu symptom (气滞血瘀证). Methods: 102 patients were randomly divided into two groups. 51 cases in the control group were treated with conventional western medicine. 51 cases in the observation group were given the Taohong Siwu decoction and TCM bone manipulation more. The levels of inflammatory factors, changes of symptom scores and improvement of lumbar function were compared between two groups before and after treatment. The efficacy in two groups was compared. Results: There was no statistical difference in the serum TNF-α, hs-CRP, IL-6 levels between two groups before treatment (P>0.05). The serum TNF-α, hs-CRP and IL-6 level in two groups were decreased (P<0.05);and those in the observation group were lower (P<0.05). The symptom scores and JOA scores in two groups did not show statistically difference before treatment (P>0.05). The symptom scores in two groups were decreased (P<0.05);and the JOA scores were increased (P<0.05). The symptom scores in the observation group were lower than the control group (P<0.05);and the JOA score was higher than the control group (P<0.05). The efficiency in the observation group was higher than the control group (P<0.05). Conclusion: The Taohong Siwu decoction and TCM bone manipulation can better reduce the level of inflammatory factors, promote the recovery of symptoms, improve the function of lumbar spine and effects, and is worthy of clinical application.
