面向分级诊疗的健康信息共享政策研究:以四川省为例

Research on Health Information Sharing Policy for Graded Diagnosis and Treatment:A case of Sichuan Province

2010年前后,推动健康信息在全国范围内共享,成为许多国家医改的重要举措。与发达国家具备成熟分级诊疗制度不同,我国是同步推进分级诊疗制度建设和健康信息共享,如何让二者相互促进、共同发展值得研究。本文以四川省为例,以分级诊疗制度建设为导向,采用信息政策领域经典的社会-技术框架,结合大数据价值链理论,深入分析四川省健康信息政策的制定和实施状况。研究发现,目前在分级诊疗、医疗机构、信息技术三个维度上存在诸多不足。针对未来发展提出如下建议:统筹推进分级诊疗制度建设与健康信息共享,以医保差异化支付为主要抓手,针对不同区域实际状况针对性引导实施健康信息共享的模式,并且打造良好的应用环境强力保障健康信息共享的顺利开展。

Around the year 2010,promoting health information sharing nationwide has become an important measure of healthcare reform in many countries.Unlike developed countries,which have mature classification and treatment systems,China is promoting the hierarchical diagnosis and treatment system as well as health information sharing simultaneously.How to realize mutual development of these two is a worthwhile subject for researchers.This paper takes Sichuan Province as an example,guided by a hierarchical diagnosis and treatment system,adopts a classic social-technical framework in the field of information policy,and combines the theory of big data value chain to deeply analyze the formulation and implementation of Sichuan's health information policy.Through study,this paper found many deficiencies in the three dimensions,namely grading diagnosis and treatment,medical institutions,and information technology.Suggestions are brought forth as follows:promote the development of hierarchical diagnosis and treatment system and health information sharing simultaneously;use differentiated payment for medical insurance as the main starting point;guide the implementation of health information sharing in accordance with actual conditions in different regions,and build a good application environment to ensure smooth development of health information sharing.
