彩色多普勒血流显像对下肢静脉血栓的诊断价值

Diagnostic value of color Doppler flow imaging in venous thrombosis of lower limbs

目的探讨彩色多普勒血流显像对下肢静脉血栓的诊断价值。方法回顾性分析2016年6月～2017年12月我院收治的疑似下肢静脉血栓患者60例的临床资料。所有患者均行彩色多普勒血流显像及超声检查诊断,对患者下肢深、浅静脉管径、血栓大小、范围、血流变化等进行测量,同时总结分析患者声像图特征。结果应用彩色多普勒血流显像诊断60例下肢静脉血栓患者,后经静脉造影证实误诊4例,彩色多普勒诊断符合率为93.33%;超声检查结果显示,中央型血栓12例,周围型34例和混合型10例,其中周围型和混合型患者中左下肢静脉形成血栓的情况明显高于右侧,差异有统计学意义(P <0.05);患侧腘静脉和股静脉管径的直径均较健侧增粗,差异有统计学意义(P <0.05);确诊的56例血栓患者中急性血栓者32例,亚急性及慢性血栓者24例。结论彩色多普勒血流显像对下肢静脉血栓的诊断率较高,且可对血栓的位置、范围和管腔阻塞程度进行判断,有助于指导临床治疗方案的制定及客观评价临床效果。

Objective To evaluate the diagnostic value of color Doppler flow imaging in venous thrombosis of lower extremities. Methods 60 patients with suspected venous thrombosis of lower extremities admitted to our hospital from June 2016 to December 2017 were retrospectively analyzed.Color doppler flow imaging in all patients were performed and ultrasound diagnosis of the patients with lower limb deep,superficial vein diameter,blood clots size,scope,and change of blood flow measurement,and summarized the characteristics analysis of patients. Results Color Doppler flow imaging was used to diagnose 60 cases of lower extremity venous thrombosis,and 4 cases were misdiagnosed by venography,the coincidence rate of color Doppler diagnosis was 93.33%.The results of ultrasound examination showed that there were 11 cases of central type thrombosis,34 cases of peripheral type thrombosis and 10 cases of mixed type thrombosis,among which the thrombosis of left lower limb vein in peripheral type and mixed type patients was significantly higher than that in the right side,and the difference was statistically significant (P < 0.05).The diameter of popliteal vein and femoral vein on the affected side was thicker than that on the healthy side, and the difference was statistically significant (P < 0.05).32 of the 56 patients with confirmed thrombosis had acute thrombus,and 24 had subacute and chronic thrombus. Conclusion The diagnostic rate of lower extremity venous thrombosis by color Doppler flow imaging is relatively high,and the location,range and degree of lumen obstruction of the thrombosis can be determined,which is helpful for the development of clinical treatment plan and objective evaluation of clinical effect.
