创面封闭式负压引流技术对开放性骨折患者生活质量和临床疗效的影响

Effect of wound vacuum sealing drainage on the quality of life and clinical outcome of patients with open fracture

目的探讨创面封闭式负压引流技术对开放性骨折患者生活质量和临床疗效的影响。方法选取我院2014年9月至2016年7月收治的开放性骨折患者100例作为研究对象,将其随机等分为对照组和观察组。对照组患者行一期清创,对骨折位置进行复位内固定或外支架固定,对创面进行长期换药和护理,待挫裂伤组织边界清楚后,进行二期创面修复。观察组患者采用创面封闭式负压引流技术。比较两组患者换药次数、创面愈合时间、生活质量评分及临床疗效。结果观察组患者换药次数少于对照组,创面愈合时间短于对照组,差异有统计学意义(P <0. 05)。干预前,两组患者生活质量评分比较差异无统计学意义(P> 0. 05);干预后,两组患者生活质量评分高于干预前(P <0. 05),观察组患者生活质量评分高于对照组(P <0. 05)。观察组患者临床疗效优于对照组,差异有统计学意义(P <0. 05)。结论采用创面封闭式负压引流技术对开放性骨折患者进行护理干预,可提高患者临床疗效和生活质量,值得临床推广应用。

Objective To explore the the effect of wound vacuum sealing drainage on the quality of life and clinical outcome of patients with open fracture. Methods To select 100 cases of patients with open fractures who were admitted to our hospital from September 2014 to July 2016, and they were divided into control group and observation group with the same number of patients randomly. Patients in the control group received first stage debridement. The fracture site was treated with internal fixation or external fixation, and wound was treated with long-term dressing and nursing. When the boundary of the contusion and laceration was clear, the second stage wound repair was conducted. Patients in the control group were treated by the wound vacuum sealing drainage technique for patients. The number of dressing changes, wound healing time, quality of life score and clinical efficacy were compared between the two groups. Results The number of dressing changes among patients in the observation group was less than that of the control group, and the wound healing time was shorter than that of the control group,and the difference was statistically significant( P <0.05).There was no significant difference in scores of quality of life between the two groups before the intervention( P >0.05). After intervention, quality of life score of the two groups were higher than those before the intervention( P <0.05).The score of quality of life in the observation group was higher than that in the control group after the intervention( P <0.05).The clinical effect in the observation group was better than that in the control group,and the difference was statistically significant( P <0.05). Conclusion Wound vacuum sealing drainage technology can improve the clinical efficacy and quality of life of patients with open fracture,which was worthy of clinical promotion and application.
