摘要
医生对坏消息传递是医疗沟通的一种重要形式。为探讨我国内地医生在坏消息传递时使用的话语策略,以及分析中西方医疗语境中坏消息传递方式的异同,文章以我国内地一部医疗题材电视剧《心术》中的医患对话为依据,采用对话分析作为理论框架,分析我国内地医生在医疗语境进行坏消息传递中如何使用话语策略,以及使用的话语策略有何特点。研究结果表明,我国内地医生在选择坏消息的告知对象时会采用直接策略与间接策略;告知方式多采用率直策略;对坏消息的解释能够采用多种解释策略;以及对坏消息传递能够总体上贯彻完全披露、同理心和诚实的原则。根据上述结果,建议我国医生对坏消息传递应总结自身优势,反思自身不足,借鉴国外优秀经验,努力创造更好的医疗沟通氛围。
Bad news delivery is an important form of medical communication.In order to explore the discourse strategies used by Chinese physicians in bad news delivery,along with the similarities and differences of bad news delivery between Chinese and Western medical contexts,this paper draws on data from a popular medical-theme TV series in China,Intention.By using the methods of Conversation Analysis,this paper tries to explore how doctors use discourse strategies when doing bad news delivery in Chinese medical contexts.Research indicates that physicians in China typically adopt both direct and indirect strategy for the choice of news recipient,use Bluntness strategy to tell the news,take different elaborating strategies,and follow the principles of full disclosure,honesty and empathy.Nevertheless,Chinese physicians should reflect on the present strategies and learn from the experience abroad for bad news delivery.
