持续性宣教管理对食管癌术后患者生活质量的影响 被引量:2

摘要 目的研究持续性宣教管理对食管癌术后患者生活质量的影响。方法选择98例食管癌术后患者为研究对象,选取就诊于2015年7月至2016年6月的患者作为常规组,选取就诊于2016年7月至2017年7月的患者作为宣教组,每组49例。术后第1天和术后4周,比较两组患者不良反应发生率;采用广泛性焦虑量表-7(GAD-7)和患者健康问卷-9(PHQ-9)评估两组患者术前和术后4周的负面情绪;采用生活质量(QOL)量表评估两组患者术前和术后4周的生活质量。结果术后4周,宣教组患者的不良反应总发生率低于常规组患者和本组术后第1天(P﹤0.05);术后4周,宣教组患者的焦虑、抑郁发生率均低于常规组患者(P﹤0.05);术后4周,宣教组患者QOL量表身体功能、生理功能、心理功能、社会功能评分均高于常规组患者(P﹤0.05),两组患者QOL量表身体功能、生理功能、心理功能、社会功能评分均高于本组术前(P﹤0.05)。结论持续性宣教管理可改善食管癌患者术后的负面情绪,降低不良反应发生率,改善生活质量。 Objective To explore the effect of continuous education management on the postoperative quality of life of patients with esophageal cancer. Method The study included 98 patients with esophageal cancer who had completed surgical treatment. Patients who were admitted to the hospital during July 2015 to June 2016 were recruited as the conventional normal control group, and those who were admitted from July 2016 to June 2017 were assigned as education group, with 49 patients in each. The incidence of adverse reactions recorded on 1st day and 4 weeks were compared between the two groups;the generalized anxiety disorder-7 (GAD-7) and the patient health questionnaire-9 (PHQ-9) were utilized to evaluate the negative emotions before surgery and 4 weeks after surgery;quality of life (QOL) scale was used to assess the quality of life score of these patients before surgery and 4 weeks after surgery. Result At 4 weeks after surgery, the total incidence of adverse reactions in education group was lower compared with normal control group, and also decreased compared with education group 1st day after surgery (P<0.05);significantly lower overall incidence of anxiety and depression were observed in education group than in normal control group (P<0.05);moreover, the physical function, physiological function, psychological function and social function scores in QOL scale of education group were consistently higher than those in the normal control group (P<0.05), and these scores in each group were improved compared with that before surgery (P<0.05). Conclusion Continuous education management can significantly improve patients’ negative emotions, and reduce the incidence of adverse reactions, bringing about better quality of life.
