奥美拉唑联合奥曲肽与胃镜辅助联合药物治疗胃出血患者的复发率差异研究

Recurrence rate of gastric bleeding treated with omeprazole alone or in combination with octreotide and drug-adjuvant gastroscopy

目的分析奥美拉唑联合奥曲肽治疗与胃镜辅助联合药物治疗胃出血患者的复发率差异。方法将我院进行治疗的63例患者作为研究对象,参考随机序列发生法产生结果对所有患者进行分组研究。其中对照组仅采用奥美拉唑治疗;观察组在对照组基础上加用奥曲肽与胃镜辅助联合药物治疗。对比2组在疗效、各项临床指标、复发率及不良反应情况的差异。结果观察组疗效94%显著高于对照组的74%(χ2=14.212,P<0.01);观察组在治疗后第1、2、3天止血率均高于对照组(第1天:72%与42%,第2天84%与61%,第3天97%与77%;P<0.05);2组经治疗后均进行为期1年的随访,观察组出血复发率9%,显著低于对照组35%(P<0.05)。结论在针对胃出血患者临床治疗时,采用奥美拉唑联合奥曲肽与胃镜辅助联合用药治疗可有效缓解患者多项临床症状,降低复发率,相较于奥美拉唑单独用药,更值得临床推广应用。

Objective To investigatethe difference in recurrence rate of gastric bleeding between treatment on omeprazole alone vs omeprazole in combination with octreotide and drug-adjuvant gastroscopy. Methods Sixty-three patients with gastric bleeding treated in our hospital were enrolled in the study. All patients were grouped according to a random table. The control group was treated with omeprazole alone, while the study group with omeprazole plus octreotide and drug-adjuvant gastroscopy. The differences between the two groups in the treatment efficacy, clinical indicators, recurrence rate and adverse reactions were compared. Results The treatment efficacy in the study group was significantly better than that in the control group (94% vs 74%,χ^2=14.212,P<0.01). The hemostasis rate of the study group was higher than that of the control group at 1 d,2 d, and 3 d after treatment (the first day 72% vs 42%,the second day 84% vs 61%,the third day 97% vs 77%;P<0.05). The two groups were followed up for 1 year after treatment. The recurrence rate of gastric bleeding in the study group was significantly lower than that in the control group (9% vs 35%,P<0.05). Conclusion In treatment of gastric bleeding, omeprazole plus octreotide and drug-adjuvant gastroscopy can effectively alleviate multiple clinical symptoms and reduce the recurrence rate, therefore worthwhile for widespread use in clinical setting compared with omeprazole alone.
