摘要
目的分析血小板分布宽度(platelet distribution width,PDW)、红细胞分布宽度(red blood cell distribution width,RDW)和红细胞沉降率(erythrocyte sedimentation rate,ESR)在结肠癌诊断及预后中的价值。方法收集2016年2月至2018年6月确诊的结肠癌患者122例,同期收取122例结肠良性疾病患者和122例健康对照。采用仪器法检测PDW与RDW,采用魏氏法检测ESR。结果单因素方差分析显示,结肠癌组、结肠良性疾病组和健康对照组间PDW、RDW及ESR水平差异均具有统计学意义(P <0. 05)。两两比较结果显示,与结肠良性疾病和健康对照组相比,PDW、RDW及ESR水平在结肠癌患者中显著升高(P <0. 05);与健康对照组相比,ESR水平在结肠良性疾病中显著升高(P<0.05)。Spearman相关性分析显示,PDW、RDW及ESR水平与结肠癌患者组织分型、分期及分化程度呈线性关联。且PDW、RDW及ESR在未分化癌、Ⅲ~Ⅳ期结肠癌及低分化结肠癌患者的增高最显著。ROC结果显示,联合PDW、RDW、ESR及癌胚抗原(carcinoembryonic antigen,CEA)区分结肠癌与结肠良性疾病和健康对照的灵敏度为92.4%,特异性为83.0%。生存分析显示,相比低PDW组,高PDW组生存率显著降低(χ~2=5.694,P=0.017);相比低RDW组,高RDW组生存率显著降低(χ~2=17.859,P<0.001);相比低ESR组,高ESR组生存率显著降低(χ~2=8.173,P=0·004)。结论联合PDW、RDW及ESR在结肠癌诊断及预后评估中具有一定意义,可为结肠癌临床诊疗提供新的方向。
Objective To analyze the value of platelet distribution width (PDW),red blood cell distribution width (RDW)and erythrocyte sedimentation rate (ESR)in the diagnosis and prognosis of colon cancer. Methods From February,2016 to June,2018,122 patients with colon cancer were collected for the study. 122 patients with benign colorectal diseases and 122 healthy controls were enrolled during the same period. PDW and RDW were detected by instrumental method,and ESR level was detected by Wechsler method. Results Univariate analysis showed that there were significant differences in PDW,RDW and ESR levels among the colon cancer group,benign disease group and healthy control group ( P <0.05). The results further showed that the levels of PDW,RDW and ESR in colon cancer patients were significantly higher than those in benign colon disease patients and healthy controls ( P <0.05),and the levels of ESR in benign colon disease patients were significantly higher than those in healthy controls ( P <0.05). Spearman correlation analysis showed that the levels of PDW,RDW and ESR were all correlated with histological typing ,stage and differentiation. Moreover,PDW,RDW and ESR increased significantly in patients with undifferentiated carcinoma,III-IV colon cancer and poorly differentiated colon cancer. ROC results showed that the sensitivity and specificity of combination of PDW,RDW,ESR and carcinoembryonic antigen (CEA)in differentiating colon cancer from benign diseases and healthy controls were 92.4% and 83.0%,respectively. Survival analysis showed that the survival rate of the high PDW group was significantly lower than that of the low PDW group (χ 2=5.694, P = 0.017 );the survival rate of the high RDW group was significantly lower than that of the low RDW group (χ 2=17.859, P <0.001);and the survival rate of the high ESR group was significantly lower than that of the low ESR group (χ 2=8.173, P =0.004). Conclusion The combination of PDW,RDW and ESR has great significance in the diagnosis and prognosis of colon cancer,and can provide a novel direction for the clinical diagnosis and treatment of colon cancer.
