自身抗体和生化检测指标在自身免疫性肝炎诊断中的价值

The exploration of the value of autoantibody and biochemical indicators in the diagnosis of autoimmune hepatitis

目的:为了提高临床对自身免疫性肝炎(autoimmune hepatitis,AIH)的检出率,分析和探讨联合自身抗体和生化指标检测的意义。方法:随机选取2015年1月至2018年1月接诊的肝炎患者130例作为观察研究对象,其中包括自身免疫性肝炎患者70例和病毒性肝炎患者60例,应用相同方法从同时间段来本院接受健康体检的人员中抽取65例作为对照研究对象,所有患者均接受自身抗体和生化指标检测,分析两组患者生化指标和自身抗体结果差异。结果:研究显示,同病毒性肝炎以及对照组相比,自身免疫性肝炎组患者其抗核抗体(antinuclear antibody,ANA)、抗平滑肌抗体(anti smooth muscle antibody,ASMA)、抗线粒体抗体(Anti mitochondrial antibody,AMA)、抗线粒体抗体M2亚型(anti mitochondrial antibody M2 subtype,AMA-M2)阳性检测率增加,组间比较差异具有统计学意义(P<0.05);自身免疫性肝炎组患者生化指标如丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、碱性磷酸酶(ALP)、谷氨酰转肽酶(GGT)等较病毒性肝炎以及对照组上升,组间比较差异具有统计学意义(P<0.05)。结论:自身免疫性肝炎患者接受自身抗体和生化检测指标联合检测,有利于提高临床确诊率,效果显著,可以应用。

Objective: To investigate and analyze the significance of detection of combined autoantibody and biochemical indicators in order to improve the detection rate of AIH (Autoimmune hepatitis) in clinic. Methods: 130 cases of patients with hepatitis treated in our hospital from January 2015 to January 2018 were selected as the subjects of the observation group, including 70 patients with autoimmune hepatitis and 60 patients with viral hepatitis according to the related requirements of the random principle and using the same method, 65 cases of people receiving health examination in our hospital from the same time period from the same hospital were selected as the control group. All patients were tested for autoantibody and biochemical indicators. The difference between biochemical and autoantibody results of two groups was analyzed by statistical software. Results: The study showed that the positive detection rate of anti nuclear antibody (ANA, Antinuclear antibody), anti smooth muscle antibody (ASMA, Anti smooth muscle antibody), anti mitochondrial antibody (AMA, Anti mitochondrial antibody) and anti mitochondrial antibody (anti mitochondrial antibody) was positive in the patients with autoimmune hepatitis,which was obviously increased, compared with viral hepatitis and the control group, and the difference between groups was statistically significant ( P <0.05). The biochemical indexes of the patients with autoimmune hepatitis, such as ALT (Alanine aminotransferase) and AST (Aspartate aminotransferase), were significantly higher than those of other patients with viral hepatitis and the patients in the control group, and there was a statistically significant difference between the groups ( P <0.05). Conclusion: The combined detection of autoantibody and biochemical indicators in patients with autoimmune hepatitis is helpful to improve the clinical diagnostic rate, and it can be applied.
