急性脑梗死患者住院费用构成及影响因素分析

Composition of Hospitalization Expenses and Its Influencing Factors in Patients with Cerebral Infarction

目的 通过对上海市某二级甲等医院2013—2016年间神经内科所有出院第一诊断为急性脑梗死的住院患者的费用进行分析,为减轻住院患者经济负担,进一步控制医疗费用过快增长提供理论和现实依据。方法 采用回顾性调查分析的方法对患者的住院费用情况、危险因素及相关检查、诊疗情况,运用SPSS19.0统计软件对住院费用进行分析。结果 根据多元线性回归分析结果表明,住院天数、年龄、治疗方式、伴随疾病个数、是否合并感染、住院次数、户籍为住院费用的主要影响因素。结论 急性脑梗死住院患者住院费用结构中药费、治疗费及检查费比例偏高,住院费用受住院天数、主诊断出院情况、年龄、是否溶栓、入院年份、使用抗菌药物、伴随疾病个数、是否本市户籍等因素影响。

Objective Through the analysis of the costs of all inpatients who were first diagnosed with acute cerebral infarction in the department of neurology of a second-level first-class hospital in Shanghai between 2013 and 2016,the theoretical and practical basis was provided to alleviate the financial burden of inpatients and further control the rapid growth of medical expenses.Methods Retrospective investigation was conducted to analyze the hospitalization cost of patients with situation,risk factors and related inspection,diagnosis and treatment,and SPSS 19.0 statistical software was used to analyze hospitalization expenses.Results According to the results of multiple stepwise regression analysis,hospitalization days,age and treatment,number of diseases,amalgamative infection,hospital number,place of birth were the main factors of hospitalization expenses.Conclusion Fees of cerebral infarction inpatients in the hospital are unreasonable.Expenses for medicine,treatment and examination proportion are on the high side,the hospitalization expenses of hospitalization days,the main diagnostic condition,age,thrombolysis,year of hospital admission,use of antibacterial drugs,number,place of birth and other factors are associated with the hospitalization expenses.
