摘要
介绍慢性病自我管理的概念、理论基础、干预等内容,为探讨适合我国国情、有效可行的慢性病自我管理模式提供参考和借鉴。
It introduced the concept,theoretical basis and intervention of chronic disease self management,in order to provide references for exploring the effective and feasible chronic disease self management model which was suitable for China′s national conditions.
