百令胶囊联合前列地尔对老年糖尿病肾病病人免疫指标及临床疗效的影响

Effect of bailing capsule combined with alprostadil on immune indexes and clinical efficacy for diabetic nephropathy in the elderly

目的探讨百令胶囊联合前列地尔对老年糖尿病肾病病人免疫指标及临床疗效的影响。方法选取我院收治的老年糖尿病肾病病人86例,按随机法将病人分为对照组和观察组,每组各43例。对照组给予前列地尔注射液治疗,观察组在对照组的治疗基础上联用百令胶囊治疗,2组均进行降压、降糖等常规治疗,比较2组治疗前后肾功能、免疫指标及临床疗效的变化。结果治疗后,2组尿素氮(BUN)、血肌酐(SCr)、尿白蛋白排泄率(UARE)水平及CD8^+均较治疗前显著降低,而CD3+、CD4^+及CD4^+/CD8^+均较治疗前显著增加(P<0. 05);观察组BUN、SCr、UARE水平及CD8^+显著低于对照组,而CD3+、CD4^+及CD4^+/CD8^+均较对照组显著增加(P<0. 05);观察组总有效率显著高于对照组,差异均具有统计学意义(P<0. 05)。结论百令胶囊与前列地尔联合用药可显著改善老年糖尿病肾病病人的临床疗效,其作用机制与改善肾功能及增强免疫能力有关。

Objective To investigate the effect of bailing capsule combined with alprostadil on immune indexes and clinical efficacy for diabetic nephropathy in the elderly.Methods Eighty-six elderly patients with diabetic nephropathy treated in our hospital were selected and randomly divided into the control group and the observation group,with 43 cases in each group.The control group was treated with alprostadil injection,and the observation group was treated with bailing capsule on the basis of the control group.The two groups received antihypertensive and hypoglycemic treatment.The changes of renal function,immune index and clinical efficacy before and after treatment were observed and compared between the two groups.Results After treatment,the levels of blood urea nitrogen(BUN),serum creatinine(SCr),urinary albumin excretion rate(UAER)and CD8^+ were significantly lower,while CD3^+,CD4^+ and CD4^+/CD8^+ were significantly higher than those before treatment in two groups.The levels of BUN,SCr,UARE and CD8^+ in the observation group were significantly lower than those in the control group,while CD3^+,CD4+ and CD4^+/CD8^+ were significantly higher than those in the control group;The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).Conclusions The combination of bailing capsule and alprostadil can significantly improve the clinical efficacy of senile diabetic nephropathy,and its mechanism is related to the improvement of renal function and immunity.
