摘要
目的分析我院就诊儿童尿路感染常见病原菌的种类及各种类耐药情况,为今后选择对尿路感染菌群敏感的抗生素提供有效依据,提升尿路感染治疗的有效率。方法采集近两年在我院临床诊断为尿路感染的患儿清洁中段尿标本203份,进行尿液常规检测及细菌培养加鉴定,并对培养出的革兰阴性菌进行产超广谱β-内酰胺酶(ex-tended-spectrum beta-lactamase,ESBLs)检测;数据进行统计分析。结果 203份标本中检出病原菌51株,检出细菌阳性率为25. 12%;在检出的51例病原菌中,大肠埃希菌为39例,占总检出菌群比为70. 58%,其次粪肠球菌为4例,占总检出菌群比为7. 84%;检出肺炎克雷伯菌为3例,占总检出菌群比为5. 88%。共有检出16例细菌产超广谱β-内酰胺酶,占总检出菌群的31. 37%,尚未检测出耐甲氧西林葡萄球菌。结论以大肠埃希菌和肺炎克雷伯菌为主的革兰阴性菌仍是儿童尿路感染病原菌主要构成,临床应根据药敏结果合理选择抗生素。
Objective Analysis of pathogenic bacteria and drug resistance in children with urinary tract infection.To provide an effective basis for the future selection of antibiotics sensitive to urinary tract infections and to improve the efficiency of urinary tract infection treatment.Methods The acquisition of nearly two years in our hospital were diagnosed with urinary tract infection in children with clean urine specimens of 203 copies,the routine urine test and bacterial culture and identification,and to cultivate the gram negative bacteria producing extended spectrum lactamases(extended-spectrum beta-lactamase,ESBLs)detection;statistical analysis.Results 203 samples of 51 strains of pathogens were detected,the positive rate of bacteria was 25.12%;in 51 cases of pathogen detection of Escherichia coli in 39 cases,the total detection ratio of 70.58%strains of Enterococcus faecalis,followed by 4 cases,the total detection ratio of 7.84%strains of Klebsiella pneumoniae were detected;in 3 cases,the total detection ratio of 5.88%bacteria.A total of 16 cases of bacteria producing broad-spectrum beta lactamase were detected,accounting for 31.37%of the total detected bacteria,and no methicillin resistant Staphylococcus was not detected.Conclusion Gram-negative bacteria,mainly Escherichia coli and Klebsiella pneumoniae,are still the main pathogens of urinary tract infection in children,and antibiotics should be selected rationally according to the results of drugsensitivity.
