消化道肿瘤肝转移介入治疗方式及临床预后分析

Interventional Therapy and Clinical Prognosis of Digestive Tract Liver Metastasis

目的研究与分析消化道肿瘤肝转移介入治疗方式及临床预后。方法选取该院2015年1月—2017年12月收治的消化道肿瘤肝转移患者192例为对象进行研究,回顾性分析其临床资料,所有患者均实施介入治疗。观察分析患者治疗效果、生存率等。结果 24例胰腺癌肝转移患者病情缓解率25.0%,1、3、5年生存率分别为66.67%、12.5%、0.0%;110例结肠癌肝转移患者病情缓解率78.18%,1、3、5年生存率分别为85.45%、34.55%、10.91%;58例胃癌肝转移患者病情缓解率82.76%,1、3、5年生存率分别为93.10%、41.38%、13.79%。其中胰腺癌肝转移患者病情缓解率、平均生存期、生存率等较结肠癌肝转移患者均显著较小,差异有统计学意义(P<0.05)。结肠癌和胃癌肝转移患者病情缓解率、平均生存期、生存率等对比差异不显著,差异无统计学意义(P>0.05)。结论针对无法根治性切除肿瘤的消化道肿瘤肝转移患者采取介入治疗的效果较为显著,其对于原发肿瘤、富血供肿瘤已切除的患者预后较好,且可提升其近五年生存率,但对于胰腺癌肝转移预后较差。

Objective To study and analyze the interventional treatment and clinical prognosis of liver metastases from digestive tract tumors.Methods A total of 192 patients with liver metastases from digestive tract tumors admitted to our hospital were enrolled in the study.The clinical data were retrospectively analyzed.All patients were treated with interventional therapy.Observe and analyze the patient's treatment effect,survival rate and so on.Results The remission rate of 24 patients with liver metastases from pancreatic cancer was 25.0%,and the 1,3,and 5 year survival rates were 66.67%,12.5%,and 0.0%,respectively;110 patients with liver metastases from colon cancer disease remission rate was 78.18%,the 1,3,and 5 year survival rates were 85.45%,34.55%,and 10.91%,respectively.The disease remission rate of 58 patients with liver metastases was 82.76%,the 1,3,and 5 year survival rates were 93.10%,41.38%,and 13.79%,respectively.The disease remission rate,average survival time and survival rate of patients with liver metastasis from pancreatic cancer were significantly smaller than those with liver metastasis from colon cancer,and the difference was statistically significant(P<0.05).There was no significant difference in the disease remission rate,average survival time and survival rate between patients with colon cancer and gastric cancer with liver metastasis,and was not statistically significant(P>0.05).Conclusion Interventional therapy for patients with liver metastases of digestive tract tumors that cannot be radically resected is more significant.It has a good prognosis for patients with primary tumors and blood-rich tumors,and can improve their survival rate in the past five years,but the prognosis for liver metastasis of pancreatic cancer is poor.
