摘要
目的观察兔膝后交叉韧带(Posterior cruciate ligament,PCL)损伤后关节内基质金属蛋白酶-2 (Matrix met-alloproteinase-2, MMP-2)活性的变化,从而探讨PCL损伤后难以自行修复的机制。方法从2018年3—8月,先将成年新西兰大白兔30只随机分成2组,对照组(假手术组)仅行右后肢髌旁内侧入路打开右膝关节后缝合切口;实验组(PCL损伤组)经右后肢髌旁内侧入路将膝后交叉韧带切断1/2后再缝合切口。然后于处理后1、2、3周分别处死每组中的5只兔,留取右后肢膝关节关节液、滑膜及PCL组织,明胶酶谱法检测标本中MMP-2的活性。结果兔膝PCL损伤后1、2周及3周,关节液中MMP-2的活性较对照组分别增加1.45倍、2.72倍及2.64倍,滑膜组织培养上清液中MMP-2的活性分别增加1.71倍、3.38倍及3.07倍,PCL组织培养上清液中MMP-2的活性分别增加0.47、2.51及2.29倍,与对照组相比差异有统计学意义(P<0.05)。结论膝关节PCL损伤后,关节液、滑膜及PCL组织中MMP-2的活性均明显增高,这可能为PCL损伤后难以自行修复可能的原因之一。
Objective To observe the changes of matrix metalloproteinase-2(MMP-2) activity in the joints of rabbits after posterior cruciate ligament(PCL) injury, and to explore the mechanism of difficult self-repair after PCL injury.Methods From March to August 2018, 30 adult New Zealand white rabbits were randomly divided into 2 groups. The control group(sham operation group) only underwent the right hind limb paramedian approach to open the right knee joint and suture the incision. The experiment group(PCL injury group) with the posterior medial approach of the right hind limb was used to cut the posterior cruciate ligament 1/2 and then suture the incision. Then, 5 rabbits in each group were sacrificed 1 week, 2 weeks, and 3 weeks after treatment. The right hind limb knee joint fluid, synovial membrane and PCL tissue were taken, and the activity of MMP-2 in the specimen was detected by gelatin zymography.Results At 1 week, 2 weeks and 3 weeks after injury, the activity of MMP-2 in synovial fluid increased by 1.45 times,2.72 times and 2.64 times compared with the control group. The MMP-2 in synovial tissue culture supernatant was observed. The activity increased by 1.71 times, 3.38 times and 3.07 times, respectively. The activity of MMP-2 in PCL tissue culture supernatant increased by 0.47, 2.51 and 2.29 times, respectively, and the difference was statistically significant(P<0.05). Conclusion The activity of MMP-2 in joint fluid, synovial membrane and PCL tissue is significantly increased after PCL injury of knee joint, which may be one of the possible reasons for difficult self-repair after PCL injury.
