颈椎椎弓根螺钉内固定应用于治疗无骨折脱位型颈脊髓损伤的疗效分析

摘要 目的分析无骨折脱位型颈脊髓损伤患者应用颈椎椎弓根螺钉内固定治疗的临床疗效。方法选取我院2016年10月~2017年10月收治的100例无骨折脱位型颈脊髓损伤患者,随机分为对照组和研究组,各50例。对照组采用保守疗法治疗,研究组采用颈椎椎弓根螺钉内固定治疗。术后随访1年,比较两组总有效率、疼痛视觉模拟(VAS评分)、颈椎神经功能(JOA)评分和并发症发生率。结果研究组总有效率为98.00%,高于对照组的80.00%,差异具有统计学意义(P<0.05)。研究组治疗1年后JOA评分高于对照组,VAS评分低于对照组,差异具有统计学意义(P<0.05)。研究组并发症发生率为4.00%,低于对照组的22.00%,差异具有统计学意义(P<0.05)。结论无骨折脱位型颈脊髓损伤患者应用颈椎椎弓根螺钉内固定治疗,疗效显著,能迅速改善患者颈椎功能,保护脊髓神经功能,缓解疼痛、活动受限等症状,术后并发症少。 Objective To analyze the clinical efficacy of cervical pedicle screw fixation in patients with cervical spinal cord injury without fracture and dislocation. Methods 100 patients with cervical spinal cord injury without fracture and dislocation were enrolled in our hospital from October 2016 to October 2017. They were randomly divided into control group and study group, 50 cases each. The control group was treated with conservative therapy, and the study group was treated with cervical pedicle screw fixation. The patients were followed up for 1 year. The total effective rate, pain visual simulation (VAS score), cervical nerve function (JOA) score and complication rate were compared. Results The total effective rate of the study group was 98.00%, which was higher than that of the control group 80.00%,the difference was statistically significant (P<0.05). The JOA score of the study group was higher than that of the control group after 1 year of treatment, and the VAS score was lower than that of the control group, the difference was statistically significant (P<0.05). The complication rate of the study group was 4.00%, which was lower than that of the control group 22.00%,the difference was statistically significant (P<0.05). Conclusion Patients with cervical spinal cord injury without fracture and dislocation are treated with cervical pedicle screw fixation. The curative effect is significant. It can quickly improve cervical spine function, protect spinal nerve function, relieve pain and activity limitation, and have fewer postoperative complications.
