HIV抗体不确定样本3种初筛结果与确证结果对比分析

Comparative analysis of three kinds of preliminary screening results and comfirmatory results of uncertain HIV antibody samples

目的通过化学发光法(CIA)、酶联免疫吸附试验(ELISA)、胶体金快速检测法(RT)3种初筛试验分析免疫印迹试验(WB)HIV抗体不确定样本,探讨4种方法检测结果的关系。方法对WB试验HIV抗体不确定的样本31例,观察WB试验结果的条带分布情况及CIA、ELISA、RT 3种初筛试验检测结果,计算CIA、ELISA的S/c.o值与WB试验结果的符合率,分析初筛试验与WB试验条带分布的关系。结果 WB试验结果的条带分布为:gp160(77.42%)、gp120(22.58%)、P24(22.58%)、p17(9.68%)、p51(3.23%)。CIA、ELISA、RT 3种初筛试验与WB试验的符合率分别为87.10%、90.32%、70.97%。3种初筛试验结果与WB试验条带分布无相关性(c2=0.859)。CIA、ELISA的S/c.o值均较低,分别为(11.14±11.03)、(5.88±3.82)。结论对于HIV抗体不确定样本,初筛试验大多为弱阳性反应并存在一定比例的漏检情况,初筛试验结果与WB试验条带分布无相关性,需引起临床重视。

Objective Analysis of uncertain HIV antibody samples in test of western blotting (WB) by means of chemiluminescence assay (CIA), enzyme linked immunosorbent assay (ELISA),rapid detection of colloidal gold (RT). To investigate the relationship of detection results by four methods. Methods The 31 samples with uncertain HIV antibody by WB test were collected. The band distribution of WB test results and three of the preliminary screening test results in CIA, ELISA and RT were observed and the coincidence rate of the S/c.o values of CIA and ELISA with WB test results were calculated. The relationships of preliminary screening test results and the band distribution of WB test results were analyzed. Results The band distribution of WB test results were: gp160 (77.42%), gp120 (22.58%), P24 (22.58%), p17(9.68%), p51(3.23%). The coincidence rate of CIA、 ELISA and RT with WB were 87.10%, 90.32%, 70.97%. It was no correlation between three of the preliminary screening test results and the band distribution of WB test results (x^2=0.859). The S/c.o values of CIA and ELISA were all low,which were (11.14±11.03) and (5.88±3.82) respectively. Conclusion For the uncertain HIV antibody samples, the most results of preliminary screening are weakly postive and exist a certain percentage of missed checks. It is no correlation between the preliminary screening test results and the band distribution of WB test results. These problems are worthy of particular mention.
