胃超声造影检查在上消化道疾病诊断中的优势及不足

Advantages and disadvantages of gastric ultrasound contrast examination in the diagnosis of upper digestive tract diseases

目的研究及分析胃超声造影检查诊断上消化道疾病的优势及不足。方法选取2016年11月至2018年6月本院收治的488例上消化道不适患者,患者均完成胃超声造影检查,对发现问题者建议同期行胃镜学检查,选取同期接受上述两种检查方法病患116例,将胃超声造影结果与胃镜检查确诊结果、病理学检查确诊结果进行比对,评价分析胃超声造影检查诊断疾病的优劣势。结果经胃镜检查确诊病患共87例,其中,胃超声造影检查分别检出反流性食管炎5例,功能性消化不良3例,复合性溃疡3例,胃溃疡9例,十二指肠溃疡7例,慢性胃炎54例,超声造影诊断符合率为90.80%;病理确诊29例,其中胃超声造影检查出胃息肉4例,胃恶性淋巴瘤2例,胃癌20例,超声造影符合率为80.66%。结论应用胃超声造影检查帮助诊断上消化道疾病具有较高符合率,可与胃镜联合应用以提高疾病的检出率。

Objective To study and analyze the advantages and disadvantages of gastric contrast-enhanced ultrasonography in the diagnosis of upper digestive tract diseases. Methods A total of 488 patients with upper digestive tract discomfort admitted to our hospital from November 2016 to June 2018 were enrolled. All patients underwent gastric sonography. The patients who found the problem were recommended to undergo gastroscopy at the same time. The results of gastric contrast-enhanced angiography were compared with the results of gastroscopy and pathological examination, and the advantages and disadvantages of gastric ultrasound contrast examination for diagnosis of disease were evaluated. Results A total of 87 patients were diagnosed by gastroscopy. Among them, 5 cases of reflux esophagitis, 3 cases of functional dyspepsia, 3 cases of complex ulcer, 9 cases of gastric ulcer and duodenum were detected by contrast-enhanced ultrasonography. There were 7 cases of ulcer and 54 cases of chronic gastritis. The coincidence rate of contrast-enhanced ultrasound was 90.80%;29 cases were confirmed by pathology, including 4 cases of gastric polyps, 2 cases of gastric malignant lymphoma and 20 cases of gastric cancer.The coincidence rate of contrast-enhanced ultrasound was 80.66%. Conclusion Gastric contrast-enhanced ultrasonography is helpful in diagnosing upper gastrointestinal diseases with high compliance rate. It can be combined with gastroscope to improve the detection rate of disease.
