血清miRNA-1246检测在早期乳腺癌诊断中的应用价值 被引量:1

摘要 目的探讨血清miRNA-1246检测在早期乳腺癌诊断中的应用价值。方法回顾性分析112例早期乳腺癌患者和104例乳腺良性病变患者的临床资料,应用荧光定量聚合酶链反应(qPCR)检测血清miRNA-1246的相对表达量,应用酶联免疫吸附试验(ELISA)检测血清糖类抗原15-3(CA15-3)、癌胚抗原(CEA)、糖类抗原125(CA125)、糖类抗原19-9(CA19-9)水平。应用Spearman法分析早期乳腺癌患者血清miRNA-1246与血清CA15-3、CEA、CA125、CA19-9表达水平的相关性,应用受试者操作特征(ROC)曲线分析miRNA-1246对早期乳腺癌的诊断价值。结果 qPCR检测结果显示,早期乳腺癌患者的血清miRNA-1246相对表达量为(1.75±0.32),明显高于乳腺良性病变患者的(1.18±0.30),差异有统计学意义(P﹤0.01)。Spearman相关分析结果显示,早期乳腺癌患者的血清miRNA-1246水平与血清CA15-3、CEA、CA125和CA19-9表达水平均呈明显正相关(r=0.646、0.573、0.721、0.597,P﹤0.01)。ROC曲线分析结果显示,miRNA-1246诊断早期乳腺癌的的ROC曲线下面积为0.783(95%CI:0.670~0.914),当血清miRNA-1246相对表达量为1.34时,约登指数最大,此时灵敏度为90.9%,特异度为70.2%。结论 miRNA-1246在早期乳腺癌患者的血清中表达水平较高,是诊断早期乳腺癌的潜在生物标志物。 Objective To explore the clinical value of serum miRNA-1246 in the diagnosis of early breast cancer.Method The study included 112 cases of early breast cancer and 104 cases of benign breast lesions,whose clinical data were retrospectively analyzed.The relative expression level of serum miRNA-1246 was detected by quantitative polymerase chain reaction(qPCR),besides,enzyme-linked immunosorbent assay(ELISA)was applied to determine the level of carbohydrate antigen 15-3(CA15-3),carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125),and carbohydrate antigen 19-9(CA19-9).Spearman analysis was utilized to investigate the association between serum miRNA-1246 and serum CA15-3,CEA,CA125,CA19-9 expression,additionally,receiver operating characteristic(ROC)curve was plotted to evaluate the clinical value of serum miRNA-1246 in the diagnosis of early breast cancer.Result qPCR indicated that,the relative expression level of miRNA-1246 in serum of patients with early breast cancer was(1.75±0.32),which was significantly higher than that of patients with benign breast lesions at(1.18±0.30),with evident difference observed(P<0.01).Spearman correlation analysis demonstrated that,the serum miRNA-1246 level in patients with early breast cancer was positively correlated with the serum expression of CA15-3,CEA,CA125 and CA19-9(r=0.646,0.573,0.721,0.597,P<0.01).ROC curve showed that,the area under the ROC curve of serum miRNA-1246 in diagnosing the early breast cancer was 0.783(95%CI:0.670-0.914),when the relative expression level of serum miRNA-1246 was 1.34,Youden index was maximized,with the sensitivity of 90.9% and specificity at 70.2%.Conclusion miRNA-1246 is highly expressed in the serum of early breast cancer and is a potential biomarker for diagnosing early breast cancer.
