对单纯白内障手术治疗合并白内障的闭角型青光眼的疗效评价

Efficacy Evaluation of Simple Cataract Surgery for Angle-closure Glaucoma with Cataract

目的观察闭角型青光眼合并白内障应用单纯白内障手术治疗的效果。方法研究对象为方便选取72例(82眼)闭角型青光眼合并白内障患者,均给予手术治疗,治疗时间2015年5月—2018年5月,依照手术方式分组,A组41眼,采用单纯白内障手术治疗,B组41眼,采用青光眼白内障联合手术治疗,观察治疗效果。结果 A组治疗总有效率为92.7%,高于对照组患者73.2%(χ~2=4.22,P<0.05)。治疗后A组视力(0.83±0.14)°,高于B组(0.54±0.22)°(t=5.15,P<0.05);A组眼压(11.3±8.7)mmHg,低于B组(19.3±10.6)mmHg(t=4.98,P<0.05);A组前房深度(3.70±0.35)mm,高于B组(1.90±0.34)mm,(t=4.65,P<0.05);A组前房粘连闭合角度(58.6±26.4)°,低于B组(168.4±89.5)°(t=9.65,P<0.05)。A组并发症发生率7.3%,低于B组29.3%(χ~22=5.22,P<0.05)。结论合并白内障的闭角型青光眼患者应用单纯白内障手术治疗时,可显著提高手术效果,提高患者术后的视力水平,改善预后情况。

Objective To observe the effect of cataract surgery on angle-closure glaucoma with cataract.Methods A total of 72 patients(82 eyes)with closed angle glaucoma and cataract were convenient enrolled.All patients were treated with surgery.The treatment time was from May 2015 to May 2018.According to the surgical procedure,41 eyes in group A were treated with simple cataract surgery.41 eyes of group B were treated with glaucoma and cataract combined with surgery to observe the therapeutic effect.Results The total effective rate of treatment in group A was 92.7%,which was higher than that in control group 73.2%(χ^2=4.22,P<0.05).After treatment,the visual acuity of group A was(0.83±0.14)°,which was higher than that of group B(0.54±0.22)°(t=5.15,P<0.05).The intraocular pressure of group A was(11.3±8.7)mmHg,which was lower than that of group B(19.3±10.6)mmHg(t=4.98,P<0.05);anterior chamber depth of group A(3.70±0.35)mm,higher than group B(1.90±0.34)mm,(t=4.65,P<0.05);group A anterior chamber adhesion closure angle(58.6±26.4)°was lower than that of group B(168.4±89.5)°(t=9.65,P<0.05).The incidence of complications in group A was 7.3%,which was lower than that in group B 29.3%,(χ^2=5.22,P<0.05).Conclusion In patients with angle-closure glaucoma with cataract,the treatment of cataract surgery can significantly improve the surgical outcome,improve the postoperative visual acuity and improve the prognosis.
