口腔正畸固定矫治器应用中牙釉质脱矿的临床分析

Clinical Analysis of Demineralization of Enamel in the Application of Orthodontic Fixed Orthodontic Appliance

目的探究口腔正畸固定矫治器应用过程中牙釉质脱矿现象。方法选择2016年11月—2017年11月我院所收治的口腔正畸固定矫治患者58例作为研究对象,通过回顾性分析当时对患者不同牙位牙齿牙釉质脱矿情况进行分析。结果 58例患者中,检出牙釉质脱矿牙齿共计93颗,总检查牙齿数为866颗,牙釉质脱矿率为10.74%,其中1级脱矿52颗(6.00%)、2级脱矿36颗(4.16%)、3级脱矿5颗(0.58%)。就不同牙位与牙釉质脱矿关系进行分析,上侧切牙、上中切牙、下侧切牙发生牙釉质脱矿比例较高,分别占比20.25%、14.10%、12.66%。纠正前,两组患者牙齿软垢指数与牙釉质脱矿指数比较,差异无统计学意义(P> 0.05);纠正后,刷牙≥3次/d患者牙齿软垢指数与牙釉质脱矿指数低于刷牙<3次/d的患者,两者比较,差异具有统计学意义(P <0.05)。结论患者在应用口腔正畸固定矫治器进行正畸过程中牙釉质脱矿发生比例较高。

Objective To explore the demineralization of enamel during the application of orthodontic fixed orthodontic appliance. Methods 58 cases of patients with fixed orthodontic treatment admitted to our hospital from November 2016 to November 2017 were selected as the study subjects. The enamel demineralization of different teeth was analyzed retrospectively. Results Among 58 patients, 93 enamel demineralized teeth were detected, and 866 teeth were examined. The demineralization rate of enamel was 10.74%. Among them, 52 teeth (6.00%) were grade 1 demineralization, 36 teeth (4.16%) were grade 2 demineralization and 5 teeth (0.58%) were grade 3 demineralization. The relationship between different tooth positions and enamel demineralization was analyzed. The incidence of enamel demineralization in upper incisors, upper central incisors and lower incisors was higher, accounting for 20.25%, 14.10% and 12.66% respectively. Before correction, there was no significant difference in the soft scale index and enamel demineralization index between the two groups (P > 0.05). After correction, the soft scale index and enamel demineralization index of patients brushing teeth more than three times a day were lower than those of patients brushing teeth less than three times a day, the difference was statistically significant (P < 0.05). Conclusion The incidence of enamel demineralization during orthodontic treatment with fixed orthodontic appliances is high.
