摘要
目的分析、研究彩色多普勒超声诊断剖宫产瘢痕妊娠(CSP的应用价值。方法选取我院2015年10月—2017年12月经术后病理证实为CSP的患者共62例作为研究对象,所有患者均采用彩色多普勒超声进行诊断,经腹诊断12例,经阴道诊断26例,经腹联合经阴道诊断24例,以病理结果为金标准,比较超声不同方式诊断CSP的符合率。结果经腹联合经阴道超声诊断CSP的符合率为95.8%,高于经阴道诊断的73.1%和经腹诊断的58.3%;误诊率为4.2%,低于经阴道诊断26.9%和经腹诊断的41.7%;超声不同方式诊断CSP的符合率对比,差异具有统计学意义(P <0.05)。结论彩色多普勒超声诊断CSP具有一定价值,采用经腹联合经阴道的检查方式可有效提升CSP诊断的准确性。
Objective To analyze and study the value of color Doppler ultrasound in the diagnosis of cesarean scar pregnancy (CSP). Methods From October 2015 to December 2017, 62 patients with CSP confirmed by pathology after operation in our hospital were selected as the study subjects. All patients were diagnosed by color Doppler ultrasonography. 12 cases were diagnosed by abdomen, 26 cases by vagina and 24 cases by abdomen combined with vagina. The coincidence rate of different diagnostic methods of ultrasound for CSP was compared with pathological results as the gold standard. Results The coincidence rate of diagnosing CSP by transabdominal combined transvaginal ultrasound was 95.8%, higher than 73.1% by transvaginal diagnosis and 58.3% by transabdominal diagnosis, and the misdiagnosis rate was 4.2%, lower than 26.9% by transvaginal diagnosis and 41.7% by transabdominal diagnosis. Comparing the coincidence rate of different diagnostic methods of ultrasound for CSP, the difference was statistically significant (P < 0.05). Conclusion Color Doppler ultrasound has a certain value in the diagnosis of CSP. Transabdominal combined transvaginal examination can effectively improve the accuracy of the diagnosis of CSP.
