回馈教学对血液肿瘤PICC置管患者握拳运动依从性与上肢静脉血流速度及导管相关并发症的影响

Effects of Teach Back on Fist Movement Adherence,Upper Extremity Venous Blood Flow Velocity and Catheter Related Complications in Hematologictumor with PICC

目的观察回馈教学对血液肿瘤经外周静脉置入中心静脉导管(PICC)置管患者握拳运动依从性、上肢静脉血流速度及并发症的影响。方法方便抽样选取南宁市某三级甲等医院血液内科一二病区2017年11月～2018年6月收治的PICC置管患者188例,按时间先后分为对照组95例与观察组93例。对照组采用常规健康教育方法,观察组采用回馈教学干预患者,观察患者3个月导管使用情况,比较两组患者握拳运动依从性、上肢静脉血流速度与导管相关并发症的发生率。结果观察组在置管后握拳运动依从性高于对照组,差异有统计学意义(P<0.05);观察组上肢静脉血流速度快于对照组(P<0.05);观察组导管相关并发症总发生率低于对照组(P<0.05)。结论回馈教学有利于提高血液肿瘤PICC置管患者握拳运动依从性,可加快上肢静脉血流速度和降低导管相关并发症发生率,可作为提高握拳运动依从性的参考依据。

Objective To examine the effects ofthe adherence of fist movement,upper extremity venous blood flow velocity and catheter related complications after teach back.Methods Totally 188 tumor patients with PICC in the department of hematology ina tertiary grade A hospital in Nanning from November 2017 to June 2018 were selected by convenient sampling and assigned into a control group ( n =95) and an intervention group ( n =93) according to chronological order.The control group were given routine health education,while the intervention group were given receivedteach back.The adherence of fist movement,venous blood flow velocity and the incidence of catheter related complications between the two groups of patients after 3 months were copored.Results The adherence of fist movement in the intervention group were significant higher than the control group ( P <0.05);Venous blood flow velocity in the intervention group were significant faster than the control group ( P <0.05);The incidence of catheter related complications in the intervention group were significant lower than the control group ( P <0.05).Conclusion Teach back can improvetheadherence of fist movement,speed up the upper extremity venous blood flowvelocity and reduce the incidence of catheter related complications,so it could provide reference for improvingthe adherence of fist movement.
