不同量表在脑卒中患者深静脉血栓形成风险评估中的预测价值

Predictive value of different scales on the risk assessment of deep venous thrombosis in stroke patients

目的研究Padua量表,Autar量表和Caprini量表在脑卒中患者深静脉血栓形成(DVT)风险评估中的预测价值。方法回顾性分析162例脑卒中患者临床资料,根据多普勒超声检查结果将患者分为DVT组和对照组,分别采用Padua量表,Autar量表和Caprini量表评估DVT发生风险,比较两组各量表评估结果,并采用受试者工作特征曲线(ROC)及曲线下面积(AUC)分析各量表对脑卒中患者DVT形成风险预测价值。结果 162例脑卒中患者出现37例DVT,发生率为22. 84%。DVT组Padua量表评分及高危患者比例高于对照组,Autar量表评分及高危患者比例高于对照组,Caprini量表评分及极高危患者比例高于对照组,差异均有统计学意义。Padua量表AUC大于Autar量表,差异无统计学意义。Padua量表AUC大于Ca-prini量表,差异有统计学意义。Autar量表AUC大于Caprini量表,差异无统计学意义。结论 Padua量表,Autar量表和Capri-ni量表用于脑卒中患者DVT发生风险评估均具有较高准确性,有助于识别DVT高危因素和采取针对性预防措施。

ObjectiveTo study the predictive value of Padua scale,Autar scale and Caprini scale on the risk assessment of deep venous thrombosis(DVT)in stroke patients.MethodsThe clinical data of 162 stroke patients were retrospectively analyzed.According to the results of Doppler ultrasonography,the patients were divided into DVT group and control group.Padua,Autar and Caprini scales were used to assess the risk of DVT occurrence.The evaluated results of the scales were compared,and the receiver operating characteristic curve(ROC)and area under the curve(AUC)were used to analyze the predictive value of each scale on the risk of DVT in stroke patients.ResultsThere were 37 cases of DVT in 162 stroke patients,and the incidence rate was 22.84%.The Padua scale score and proportion of high-risk patients in DVT group were higher than those in control group.The ratio of Autar scale and high-risk patients was higher than that of control group,and the Caprini scale score and proportion of extremely high-risk patients were higher than those in control group.The AUC value of Padua scale was greater than that of Autar sclae and Caprini scale,and the difference in the former was not statistically significant,and the difference in the latter was statistically significant,and the AUC value of Autar scale was greater than that of Caprini scale.ConclusionPadua,Autar and Caprini scales all have high accuracy in the assessment of DVT risk in stroke patients,and they are helpful for identifying DVT high-risk factors and adopting targeted preventive measures.
