摘要
目的探讨桥接组合式内固定系统与传统锁定加压钢板治疗对股骨干骨折患者围术期指标、骨折愈合时间及并发症的影响。方法将96例股骨干骨折患者按随机数字法分为观察组和对照组各48例。观察组予桥接组合式内固定系统治疗,对照组予传统锁定加压钢板治疗。比较两组围术期指标、骨折愈合时间、住院时间、髋关节功能、疼痛程度、手术疗效、并发症发生率等指标。结果两组切口长度比较差异无统计学意义(P> 0. 05);观察组手术时间、住院时间、骨折愈合时间短于对照组,术中出血量、术后引流量低于对照组,术后髋关节活动度大于对照组,Harris评分高于对照组,视觉模拟评分法(VAS)评分、术后并发症发生率低于对照组(P <0. 05);末次随访,两组手术优良率比较差异无统计学意义(P> 0. 05)。结论与传统锁定加压钢板治疗股骨干骨折比较,桥接组合式内固定系统治疗可减少手术损伤,缩短手术时间、住院时间和骨折愈合时间,促进髋关节功能恢复,减轻术后疼痛,降低并发症发生率,但两者最终均能达到较高的手术疗效。
Objective To explore the effects of bridge combined fixation system and traditional locking compression plate on perioperative indexes,fracture healing time and complications in patients with femoral shaft fractures.Methods Ninety-six patients with femoral shaft fractures were divided into observation or control group,48 in each group according to the computer random number method.The observation group was treated with bridge combined fixation system,while the control group was treated with traditional locking compression plate.The perioperative indexes,fracture healing time,hospital stay,hip joint function,pain level,surgical efficacy and incidence of complications were compared between the two groups.Results There was no significant difference in incision length between the two groups(P>0.05).The operative time,hospital stay and fracture healing time in the observation group were shorter than those in the control group,and the intraoperative blood loss and postoperative drainage volume were lower than those in the control group(P<0.05).The hip range of motion after operation was higher and the Harris score were higher while the YAS score and incidence of postoperative complications were lower in the observation group than those in control group(P<0.05).There was no significant difference in the excellent and good rate of surgery between the two groups at the last follow-up(P>0.05).Conclusion Compared with traditional locking compression plate for the treatment of femoral shaft fractures,bridge combined fixation system can not only reduce the surgical injury,and shorten the operative time,hospital stay and fracture healing time,but also promote the hip function recovery,and reduce the postoperative pain and the incidence of complications.Even so,both methods can eventually achieve high surgical efficacy.
