急性期脑梗死患者血清UCH-L1和GFAP水平及其与病情和神经功能缺损程度的相关性研究 被引量:31

摘要 目的探讨急性期脑梗死患者血清泛素羧基末端水解酶-1(UCH-L1)和胶质纤维酸性蛋白(GFAP)水平及其与病情和神经功能缺损程度的相关性。方法我院收治的98例急性期脑梗死患者(观察组)和60例同期我院体检正常者(对照组),所有研究对象均记录性别、年龄等一般临床资料和检测血清UCH-L1和GFAP水平,观察组均行头部CT或磁共振检查和神经功能评定,比较不同急性期脑梗死患者间上述资料的差异性。结果观察组血清UCH-L1、GFAP水平均高于对照组(P<0. 05);不同病情和不同神经功能缺损程度的急性脑梗死患者间血清UCH-L1、GFAP水平比较差异有统计学意义,且随着梗死病灶越大和神经功能缺损程度越严重,血清UCH-L1、GFAP表达水平越高(P <0. 05);血清UCH-L1、GFAP的表达水平与梗死灶直径大小和NIHSS评分均呈正相关(P <0. 05)。结论急性期脑梗死患者血清UCH-L1和GFAP水平均显著上升,且与其梗死灶体积和神经功能损伤程度均呈正相关,可用来作为临床上评估急性脑梗死患者病情和神经功能损伤的有效指标。 Objective To investigate the association of serum levels of UCH-Ll and GFAP with patient’s condition and degree of nervous functional defects during acute period of cerebral infarction.Methods Ninety-eight patients with acute cerebral infarction in our hospital were taken as observation group,and 60 healthy subjects at the same time were taken as control group.The general clinical data such as gender,age,and serum levels of UCH-L1 and GFAP were recorded.The patients underwent brain CT or MRI and neurological evaluation.The differences in the data during various acute periods of cerebral infarction were compared.Results Serum levels of UCH-L1,and GFAP in the patients were significantly higher than those in the control group(P<0.05).There was statistic significance in the serum levels of UCH-L1 and GFAP among patients with different conditions and neurological impairments(P<0.05).With the increase of the infarction and the extent of the neurological defect,the serum levels of UCH-L1 and GFAP were more increased(P<0.05).Serum levels of UCH-L1 and GFAP were related to size of infarction and NIHSS score(P<0.05).Conclusion The Serum levels of UCH-L1 and GFAP in patients with acute cerebral infarction are significantly increased.The levels are positively correlated with infarction and neurological impairment.Therefore,the two indexes can be used as an effective marker for evaluating the condition and degree of nervous functional defects during acute period of cerebral infarction.
